血清Cys C、RBP、Hcy和hs-CRP联合检测在糖尿病肾病诊断中的应用

摘    要:目的研究血清中胱抑素C(CysC)、视黄醇结合蛋白(RBP)、同型半胱氨酸(Hcy)、超敏C反应蛋自(hs—CRP)联合检测在糖尿病肾病(DN)诊断中的运用。方法研究190例DM患者与50例健康对照者的血清CysC、RBP、Hcy、hs—CRP浓度变化。CysC、RBP、hs—cRP血清浓度采用胶乳颗粒增强免疫比浊法测定,Hcy血清浓度采用循环酶法测定。结果DM患者正常白蛋白尿组的血清CysC、RBP、Hcy、hs—CRP浓度比正常对照组明显升高(P〈0.05),DM患者微量白蛋白尿组和临床蛋白尿组较对照组血清CysC、RBP、Hcy浓度显著升高(P〈0.01),血清CysC、RBP、Hcy、hs—CRP浓度随糖尿病肾病(DN)的发生及发展严重程度逐渐增高。结论清CysC、RBP、Hcy和hs—CRP联合检测可作为诊断早期糖尿病肾病较为敏感的指标;对监测早期DN的发生和病情发展程度有重要的意义。

Diagnostic application of the joint detection of serum Cys C,RBP, Hcy and hs-CRP in patients with diabetic nephropathy

Abstract:Objective To study the diagnostic value of the combined detection of serum cystatin C (Cys C),Retinol Binding Protein (RBP),homocysteine (Hcy) and high sensitivity C-reactive protein (hs-CRP) in diabetic nephropathy. Methods The changes of serum Cys C ,RBP,Hcy, and hs-CRP in 190 cases of DM patients and 50 healthy control cases were compared. The serum Cys C ,RBP and hs-CRP were conducted by using latex particle enhanced im- munoturbidimetric assay, the serum Hey was conducted by using cycle enzymatic assay. Results DM patients with normal albuminuria group, serum Cys C,RBP, Hcy and hs-CRP concentration were significantly higher than those of normal control group(P〈0.05),serum Cys C and Hcy concentration were higher significantly (P〈0.01) in DM patients with microalbuminuria and clinical proteinuria than normal control group,thc concentration of serum Cys C, RBP, Hey and hs-CRP gradually increased with the occurrence and severity of diabetic nephropathy. Conclusion The detection of serum Cys C, RBP, Hey and hs-CRP could be used as a sensitive indicator for early diagnosis of DN and monitoring the occurrence of early DN and the degree of progression of the disease.
