整体护理在糖尿病肾病血液透析患者中的效果分析

目的 探讨糖尿病肾病血液透析患者实施全面整体护理后的临床效果.方法 选取2012年3月～2013年9月来我院进行血液透析治疗的糖尿病肾病患者118例,按照实施的护理方法分为对照组及观察组,每组59例,分别实施常规护理、全面整体护理,比较两组护理前后SCL-90评分、生活质量得分、存活状况及并发症发生情况.结果 对照组及观察组患者护理后的SCL-90量表中的焦虑、忧郁、躯体症状、敌对情绪及强迫症评分均低于护理前,观察组护理后评分低于同期对照组（P＜0.05）.对照组及观察组患者的生活质量表中的一般情感指数、情感得分、健康指数及生活功能指数得分均高于护理前,观察组护理后得分高于同期对照组（P＜0.05）.观察组患者存活1年、存活3年及存活5年的存活率与对照组比较均较高（P＜0.05）.观察组患者的并发症中低血压、高血压、心力衰竭、感染发生率与对照组比较较低（P＜0.05）.结论 糖尿病肾病血液透析患者实施全面整体护理能够提高患者的生活质量,降低并发症发生率,使患者的生存期限有效延长.

Effect of holistic nursing in hemodialysis patients with diabetic nephropathy application

Objective To investigate the clinical effect of diabetic hemodialysis patients after the implementation of holistic nursing. Methods A total of 118 cases with diabetic nephropathy were selected from March 2012 to September 2013 in our hospital for hemodialysis patients, they were divided into control group and observation group in accor- dance with the implementation methods of care, there were 59 cases in each group, given implemented routine care, holistic nursing,respectively. We compared the SCL-90 score,quality of life score,survival status and occurrence of complications before and after nursing. Results SCL-90 scale of anxiety, depression, somatization, obsessive-compul- sive disorder and hostility scores of the control group and the observation group after care were lower than before treat- ment,the score of observation group after care was lower than that of the control group （P〈0.05）.General sentiment,e- motion score, health indices and life care function index scores in the patients＂ quality of life table of Control group and observation group after care were higher than before care ,the score of observation group after care were higher than that of the control group on the same period （P〈0.05）. Survived 1 year, 3-year survival and survival five year survival rate in the observation group were higher than that in the control group （P 〈0.05）. Hypotension, hypertension, heart failure, the incidence of infection in the observation group were lower than those in the control group （P 〈0.05）. Conclusion Hemodialysis patients with diabetic nephropathy implement holistic care can improve the quality of life of patients and reduce the incidence of complications, the patient＇s survival period is effectively extended.
