颈臂丛联合阻滞麻醉在锁骨骨折手术中的应用

目的 观察颈臂丛联合阻滞麻醉在锁骨骨折手术中的效果.方法 选择我院2010年8月～2013年8月共60例锁骨骨折患者实施骨折切开复位内固定术,将其随机分为实验组和对照组,每组30例.实验组实施颈臂丛联合阻滞麻醉,对照组实施颈丛改良法阻滞麻醉,对阻滞效果进行评估.结果 实验组的优良率为100％,对照组为83.3％,实施颈臂丛联合阻滞麻醉效果明显高于颈丛改良法阻滞麻醉,差异有统计学意义（P＜0.05）.实验组出现4例轻度的霍纳式综合征,1例喉反神经阻滞;对照组出现6例霍纳式综合征,3例喉反神经阻滞,差异有统计学意义（P＜0.05）,无膈神经阻滞、局麻药中毒等不良反应.结论 颈臂丛联合阻滞麻醉是一种效果非常理想的麻醉方法,为手术顺利进行创造了良好的条件.

Neck brachial plexus block anesthesia in the application of the clavicle fracture surgery

Objective To observe the effect of neck brachial plexus block anesthesia in clavicular fracture. Methods From August 2010 to August 2013, a total of 60 patients with fracture of clavicle implementation of open reduction and internal fixation of fracture, were randomly divided into experiment group and the control group, 30 cases in each group.Experiment gorup was applied neck brachial plexus block anesthesia, control group was applied the modified method of cervic plexus block anesthesia, evaluate blocking effect were compared. Results The excellent and good rate of the experiment group was100%, while the control group was 83.3% ,the implementation of neck brachial plexus block anesthesia effect was significantly higher than the modified method of cervical plexus block anesthesia, the dif- ference was statistically significant （P 〈 0.05）. Experiment group in 4 cases of mild homer syndrome, 1 case of throat against nerve block; and the control group in the homer syndrome type 6 cases, 3 cases of laryngeal nerve block, the difference was statistically significant （P 〈 0.05）,without phrenic nerve block, local anesthetics poisoning adverse reaction. Conclusion Neck brachial plexus block anesthesia is a kind of anesthesia method with ideal effect, created good conditions for the smooth operation.
