奥曲肽联合加贝酯预防ERCP术后高淀粉酶血症及胰腺炎的疗效观察

目的 观察奥曲肽联合加贝酯预防ERCP术后高淀粉酶血症及胰腺炎的疗效.方法 选取2011年1月～2014年1月在我院行ERCP治疗的患者60例,随机分为观察组（n=30）与对照组（n=30）,两组均同时行抗炎、补液等对症支持治疗,其中观察组同时予奥曲肽联合加贝酯治疗,比较两组胰腺炎、高淀粉酶血症的发生率,以及两组患者ERCP术前术后不同时点血清淀粉酶的变化情况.结果 观察组与对照组患者胰腺炎、高淀粉酶血症的发生率分别比较,差异有统计学意义（P＜0.05）.术后3h两组患者的血淀粉酶水平较术前显著升高,但观察组升高幅度显著低于对照组（P＜0.01）;术后24 h观察组患者血清淀粉酶水平显著低于对照组（P＜0.01）.结论 奥曲肽联合加贝酯用于预防ERCP术后高淀粉酶血症及胰腺炎疗效确切,可以明显降低血淀粉酶水平,值得推广和应用.

Efficacy of octreotide and gabexate prevention of post-ERCP hyperamylasemia and pancreatitis

Objective To observe the efficacy of octreotide and gabexate prevention post-ERCP hyperamylasemia and panereatitis. Methods Sixty patients treated in our hospital were randomly divided into two groups （n=30） and control group（n=30）, both groups were given simultaneously anti-inflammatory, rehydration symptomatic and supportive treat- ment, while the observation group combined oetreotide and gabexate,the incidence of pancreatitis and hyperamy- lasemia, and serum amylase of at different time points after surgery ERCP were compared. Results The incidence of hyperamylasemia and panereatitis was statistically significant between two groups （P〈0.05）. The serum amylase level was significantly higher than the preoperative after ERCP 3h, the observation group was significantly lower than the control group （P〈0.01）;After 24h, the serum amylase levels of the observation group were significantly lower than the control group （P〈0.01）. Conclusion Octreotide and gabexate prevention of post-ERCP panereatitis hyperamylasemia have good efficacy, can significantly reduce serum amylase level, worthy of promotion and application.
