猪脱细胞真皮基质在人皮肤创伤修复中的临床应用

摘    要:目的探讨应用猪脱细胞真皮基质覆盖深Ⅱ度烧伤削痂后的创面。方法将8(j例烧伤总体表面积(totalbodysurfacearea,TBsA)1%~93%病例随机分为两组,猪脱细胞真皮基质(治疗组)40例,传统保痂组(对照组)40例。观察两组创面愈合时间和愈合质量及并发症情况,治愈患者经过2个月~2年的随访,观察疤痕增生情况。结果治疗组中途创面不需换药,创面愈合时间缩短,平均(13.8±2.6)d,而对照组愈合时间(18.2±3.8)d,差异具有统计学意义(P〈0.05)。同时,治疗组疤痕增生较对照减轻或无疤痕。结论猪脱细胞真皮基质治疗覆盖Ⅱ度烧伤削痂后的创面疗效显著,能加快创面愈合,减轻疤痕增生,降低烧伤感染和炎症反应综合症的发生,同时减轻病人的经济负担。

Abstract:Objective To study porcine acellular dermal matrix for covering deep partial thickness burn wound after escharectomy. Methods Eighty patients with 1%-93% total body surface area (TBSA) burn were divided into two groups, i.e. treatment group (40 eases, the wound was covered with porcine acellular dermal matrix) and control group (40 cases, undergoing traditional eschar reservation treatment). Time and quality of wound healing and complications were observed. After 2 months to 2 years follow up, scar proliferation was observed. Results Treatment group did not need any more dressing change. Healing time of the wound were significantly different ( 13.8 ±2.6)d versus (18.2 ± 3. 8)d,P〈0. 05] between two groups. Treatment group had milder scar proliferation than control group. Conclusion Porcine acellular dermal matrix has notable therapeutic effect on deep partial thickness burn wound, can accelerate wound healing, lighten scar proliferation, decrease the occurrence of burn infection and inflammatory response syndrome, and reduce the economic burden of the patients.
