85例早发型重度子痫前期终止妊娠时机的探讨

目的探讨早发型重度子痫前期对围生期母婴预后的影响。方法对85例早发型（孕28-34周）重度子痫前期的患者临床资料进行回顾性分析,根据其期待治疗后的终止妊娠时间不同分为三组：A：28-32^＋6周23例,B：33-34^＋6周46例,C：〉35周16例。结果早发型重度子痫前期患者期待治疗平均延长孕周（11.5±6.9）d,围生儿死亡率随发病孕周的延长而显著降低,差异有统计学意义（P〈0.05）,并发症发生率随发病孕周增长而降低。结论早发型重度子痫前期严重影响母儿预后,适当的期待治疗有助于改善围产儿预后及减少孕产妇并发症。并在期待治疗过程中,应严密监护母胎情况;选择适当孕周终止妊娠。

Study on Gestational Termination Time of 85 Cases of Pregnancy with Early Onset Severe Preeclampsia

Objective To explore the effect of early onset severe preeclampsia on the maternal and neonatal outcomes. Methods The clinical data of 85 patients with early onset severe preeclampsia were retrospectively analyzed.All the patients were divided into three groups based on gestational termination time after expectant treatment: group A(28～32+6 weeks,n=23),group B(33～34+6 weeks,n=46),and group C(> 35 weeks,n=16). Results The median overall prolongation of gestation was(11.5±6.9) days for all patients.The neonatal mortality rate was significantly decreased with the extension of gestational age,and the difference was statistically significant(P<0.05).The incidence rate of pregnant complications in early onset severe preeclampsia was declined along with the postponement of pathogenic week of gestation. Conclusion The early onset severe preeclampsia badly influences the maternal and neonatal outcomes.Proper expectant treatment is conducive to improving the prognosis of perinatal infants and reducing maternal complications.During the course of expectant treatment,the situation of maternity and fetus should be closely monitored.The termination of pregnancy should be done in good time.
