胃癌患者手术前后血清TSGF、AFP及HER-2/neu ECD的变化与意义

目的探讨胃癌患者手术前后血清TSGF、AFP及HER-2/neu ECD的变化及意义,为肿瘤标志物水平预测手术预后提供临床依据。方法对本院82例行胃癌根治术的患者进行回顾性分析：（1）分析术后3个月、6个月、1年的血清肿瘤特异生长因子TSGF、甲胎蛋白AFP及表皮生长因子受体HER-2/neu ECD降低情况;（2）不同临床病理参数的TSGF、AFP及HER-2/neu ECD降低情况;（3）肿瘤标志物不同降低水平的胃癌复发和累计生存率的影响。结果 （1）术后3个月后,血清TSGF、AFP及HER-2/neu ECD水平均降低,且与术后6个月、1年差异有统计学意义;（2）除了年龄外,其余病理参数在同一分级的降低情况差异均有统计学意义;（3）除了术后3个月外,低降低组的复发率高于高降低组,且术后900 d累计生存率低于高降低组,除TSGF（P〈0.01,Log-rank检验）,其余两个指标为P〈0.05。结论血清TSGF、AFP及HER-2/neu ECD手术前后的变化不仅可用来评价治疗效果,也可用来衡量术后的复发和生存情况。

The Changes and Significance of Serum TSGF, AFP and HER-2/neu ECD Levels in Patients with Gastric Cancer Before and After the Operation

Objective To explore the change of serum TSGF,AFP and HER-2/neu ECD level of patients with gastric cancer before and after the operation,as to provide clinical information for tumor markers to predict the prognosis postoperative. Methods A retrospective analysis was made on 82 patients with gastric cancer having radical gastrectomy:(1) The decreased situation of serum TSGF,AFP and HER-2/neu ECD level of 3rd month,6th month and 1st year postoperative.(2) The decreased situation of serum TSGF,AFP and HER-2/neu ECD level of different clinical pathology parameters.(3) The recurrence and accumulative survival rate of different decreased situation of tumor markers. Results (1) The third month postoperative,the level of serum TSGF,AFP and HER-2/neu ECD all decreased having significant differences with 6th month and 1st year postoperative.(2) Except the age,the remaining pathology parameters all showed significant differences on the same category.(3) Except 3rd month postoperative,the recurrence of hypo-reducing group was superior to that of hyper-reducing group,meanwhile the 900 days accumulative survival rate showed the same trend,such as TSGF(P<0.01,log-rank test),AFP(P<0.05,log-rank test),and HER-2/neu ECD(P<0.05,log-rank test). Conclusions The change of serum TSGF,AFP and HER-2/neu ECD levels both before and after the operation can not only be used to evaluate the effect of the operation,but also can measure the recurrence and survival rate.
