地佐辛或芬太尼硬膜外预先镇痛对硬膜外麻醉效果影响的比较研究

摘    要:目的 观察硬膜外注入地佐辛或芬太尼施行预先镇痛对硬膜外麻醉效果的影响.方法 妇科手术患者96例,于硬膜外麻醉后随机分成3组,每组32例.芬太尼组于切皮前经硬膜外注入芬太尼与氟哌啶;地佐辛组于切皮前经硬膜外注入地佐辛与氟哌啶.对照组经硬膜外注入生理盐水.观察术中镇痛效果,腹肌松弛质量以及不良反应.结论 芬太尼组和地佐辛组的硬膜外麻醉和肌松效果相当,此两组的效果优于对照组(P〈0.05),三组不良反应差异无统计学意义.结论 切皮前15 min经硬膜外注射芬太尼或地佐辛施行预先镇痛,均可有效增强硬膜外麻醉效果.

Abstract:Objective To observe the influence of preemptive analgesia with epidural dezocine or fentanyl on the epidural anesthesia. Metheds Ninety-six patients with gynecologic surgery under epidural anesthesia were randomly divided into three groups, In fentanyl group (n =32) fentanyl 4μg/kg and droperidol 2 mg was injected via epidural catheter 15 min before skin incision. In dezocine group (n=32) dezocine 0.1 mg/kg and droperidol 2 mg was injected via epidural catheter 15 rain before skin incision. In control group (n=32) only normal saline of equal volume was injected. Quality of analgsetic effect and abdominal muscle relaxation were assessed, and the advevse effects were observed. Results The analgesia effect and the abdominal muscle relaxation in the two preemptive analgesia group were similar,which were better than those in control group(P〈0.05). The adverse effects were no significant difference in three groups. Conclusion Epidural preemptive analgesia with dezocine or fentanyl via epidural injection can effectively improve the epidural analgesia effect.
