肛管直肠恶性黑色素瘤的诊断和治疗(附8例临床分析)

目的：分析肛管直肠恶性黑色素瘤（MAR）的临床特征，探讨诊治要点和减少误诊误治的措施。方法：对我院1976－1997年间经手术及效理证实的8例MAR临床特征和治疗情况作回顾性分析。结果：8例中6例有不同程度使血，大便习惯改变，骶尾部及肛周疼痛等直肠肛门症状，1例为肛门部慢性溃疡，1例表现为腹股沟包块，术前均误诊为血栓痔，陈旧性肛裂，肛瘘等常见肛直肠疾病。结论：必须提高对MAR的认识水平，了解不同类型MAR的临床特征，才能正确诊治，减少误诊误治。

The diagnosis and treatment of malignant melanoma in anorectal region.

Objective To analyze the clinical features of malignant melanoma in anorectal region(MAR) and make an approach to the main points of the diagnosis and treatment of this disease.Methods Eight cases of MAR confirmed by surgery and pathology from 1976 through 1997 were analyzed retrospectively.Results Of the 8 patients,6 had hematochezia in various degrees and change in bowel habit,and they suffered a pain in the sacrococcyx and the surroundings of anus;1 patient had chronic ulcer in anal region;1 patient suffered from a mass in the groin.All of these cases had been misdiagnosed as the common lesion of anorectal region with hemorrhoids,thrombosed hemorrhoids,fissure and fistula.Conclusion We must heighten our awareness of MAR,understand its types and clinical features so as to make correct diagnosis and avert misdiagnosis.
