肘管综合征的神经电生理结果56例分析

摘    要:目的通过分析56例肘管综合征神经电生理检测结果,探讨神经电生理检测对肘管综合征的定位诊断及鉴别诊断的价值。方法选取56例确诊为肘管综合征的患者,测定尺、正中运动神经及感觉神经传导速度,第一骨间肌、小指展肌、尺侧腕屈肌、拇短展肌肌电图。记录神经传导速度、诱发电位波幅、波形的离散度、远端潜伏期、支配肌有无失神经表现。正常值参照本实验室不同年龄组正常值2个标准差。结果56例肘管综合征患者所测正中运动及感觉神经传导速度均正常,拇短展肌均未见神经原性损害。尺神经运动神经肘上-肘下传导速度减慢54例(96.4%),与自身健侧对比传导速度减慢2例(3.6%),尺神经肘上-肘下-腕传导速度均减慢10例(17.9%),尺神经感觉神经诱发电位波幅下降42例(87.5%),诱发电位消失14例。伴随肌电图异常38例(67.9%),其中第一骨间肌、小指展肌异常38例,尺侧腕屈肌异常9例(16.1%)。肘上-肘下神经传导速度异常率明显高于其他指标,诱发电位波幅、波形离散度、肌电图的变化与病变程度密切相关。结论神经电生理检测对肘管综合征定位、鉴别诊断有重要意义。

Abstract:Objective To evaluate the detection and significance of nerve electrophysiotogical examination on Cubital tunnel syndrome. Methods Totally 56 cases with Cubital tunnel syndrome were selected and the nerve electrophysio logical examination was used to them. Results There were 54(96. 4% ) cases shown Ulnar motor nerve conduction ve locity across the elbow lower than normal. There were 42 (87.5%) cases shown sensory nerve potentials amplitudes re duced and 14 cases evoked potentials disappeared. There were 38 cases appeared abnormal electromyography, among which 38 cases shown abnormal electromyography on first dorsal interosseous muscle and Abductor Digiti Minimi Muscle and 9 (16. 1% ) cases on Flexor Carpi Ulnaris. Conclusion Nerve electrophysiological examination is helpful to the nerve damage location and the differential diagnosis of cubital tunnel syndrome.
