摘    要:目的:探讨降钙素基因相关肽在消化性溃疡发病机制中的作用。方法:本文采用放免法测定了46例胃十二指肠溃疡病患者、15例正常对照的血浆及组织中CGRP的含量和其与溃疡面积的相关性。结果:在各溃疡组血浆CGRP的浓度与正常组比较均降低。溃疡活动期血浆CGRP浓度较低,与对照组比较有显著性差异 P < 0.01。愈合期含量较高,疤痕期浓度逐渐恢复,与对照组比较无显著性差异P > 0.05。溃疡边缘粘膜组织中CGRP的含量明显低于自身正常粘膜组织及对照组粘膜组织中的含量,且粘膜组织中CGRP的含量与溃疡面积无相关性(r = 0.2526, P >0.05)。结论: 本组试验结果提示,降钙素基因相关肽在消化性溃疡的修复中可能起着重要的作用。且低水平的降钙素基因相关肽可能是溃疡形成的因素之一。

Abstract:Objective: To study the effect of the calcitonin gene-related peptide on the pathogenesis of peptic ulcer. Methods: In this article, the concentrations of calcitonin gene-related peptide in gastric mucosa and peripheral plasma were assessed by radioimmunoassay in 46 patients with peptic ulcer and 15 healthy adults. The relationship between CGRP concentrations in plasma or mucosa and the area of ulcer was examined. Results: The results showed that plasma calcitonin gene-related peptide concentrations were reduced (P < 0.01 compared with normal) in the ulcer groups. Plasma CGRP concentrations in active stages of ulcer were significantly less than those in normal subjects, and those in healing stages were significantly greater than those in normal subjects (P < 0.01), and those in scarring stages of ulcer were gradually restored .CGRP level of ulcer mucosa was different from CGRP level of its normal mucosa and the mucosa of control groups (2.5289 2.576 pmol/L/g; 7.374 6.7045 pmol/L/g, P < 0.01; and 5.4515 1.7784pmol/L/g, P < 0.01).There was no correlation between the calcitonin gene-related peptide concentrations in mucosal specimens from ulcer margins and the area of ulcer (r = 0.2526, P > 0.05). Conclusion: Our studies suggest that CGRP might participate in a restoration mechanism of ulcer; and locally reduced calcitonin gene-related peptide may be an important factor contributing to the pathogenesis of peptic ulcer.
