胎儿宫内生长迟缓脐血流频谱波形变化

摘    要:应用彩色连续波超声多普勒血流分析仪对312例孕晚期孕妇共进行363次脐血流检查,29例胎儿宫内生长迟缓(IUGR)者脐血流S/D比值(3.58±1.16)、PI(脉动指数,1.17±0.29)、RI(阻力指教,0.73±0.18)较正常组(分别为S/D比值2.68±0.76、PI0.92±0.22、RI0.60±0.09)明显增高(P<0.01);FBVR(快速血流量比,0.51±0.06)较正常组(0.57±0.05)明显降低(P<0.0005)。这些变化可能与胎儿心脏射血功能下降和/或胎盘血管阻力增高有关。FBVR作为判断血流的新指标,在IUGR与正常组比较时,差异的显著程度较S/D比值、PI、RI明显。1例胎死宫内的严重IUGR胎儿,死亡前表现为舒张末期血液断流及心律不齐现象,可见舒张末期血流断流是胎儿危险的征象。

Abstract:Proliferative diabetic retinopathy(PDR)is one of the main causes of blindness in the world.Although plenty of studies have been done,the pathogenesis of it remains unclear.The present study investigated the relationship of intercellular adhesion molecules,ICAM 1 and VCAM 1,with PDR by means of immunohistochemical techniques.The results showed that ICAM 1 was detected in 36 out of 40(90%)PDR epiretinal membranes,which were obtained surgically from 40 patients with proliferative diabetic retinopathy,and VCAM 1 was found in 32 out 40 cases(80%).The positive cells were mainly lymphocytes,vascular endothelial cells,fibroblasts,and retinal pigment epithelial cells.The expression of cell adhesion molecules,especially ICAM 1 and VCAM 1 in diabetic epiretinal membranes suggests that cell to cell interactions and immune responses may play a significant role in the devlopment of PDR.
