甲状腺手术使用超声刀与甲状旁腺保护

目的探索在甲状腺全切或近全切术中使用超声刀是否利于保护甲状旁腺避免损伤。方法 147例分别使用超声刀行或传统方法行甲状腺全切或近全切术,比较两者在术中对甲状旁腺保护和术后甲状旁腺功能低下的发生情况,评估超声刀在术中甲状旁腺保护的优缺点。结果使用超声刀组84例,术后8例出现暂时性甲状旁腺功能低下,传统手术组63例,术后15例出现暂时性甲状旁腺功能低下,2例出现永久性甲状旁腺功能低下。结论甲状腺全切或近全切术中合理利用超声刀有助于保护甲状旁腺血供,减少术后甲状旁腺功能低下的发生几率。

Association of the Use of Harmonic Scalpel in Thyroidectomy with Parathyroid Gland Conservation

Objective To investigate whether the use of harmonic scalpel in total and subtotal thyroidectomy was beneficial for parathyroid gland conservation or not. Methods 147 patients underwent total or snbtotal thyroideetomy with harmonic scalpel or conventional hemostasis, the incidences of hypoparathyroidism after operations were compared between two groups. Results In 84 patients who underwent thyroidectomy with harmonic scalpel,8 cases presented as transient hypoparathyroidism after operations. In 63 patients who underwent thyroidectomy with conventional hemostasis, 15 cases presented as transient hypoparathyroidism while 2 ones presented as eternal hypoparathyroidism after operations. Conclusion The use of harmonic scalpel in total and subtotal thyroidectomy was beneficial for protection on parathyroid gland and its artery supplies, it was useful to prevent hypoparathyroidism after surgery.
