血清碱性磷酸酶对判断肝硬化病情及预后的临床价值

目的:分析研究肝硬化患者血清碱性磷酸酶(ALP)与肝纤维化标志物及Child pugh分级的关系。方法:检测5 6例肝硬化患者的血清ALP、胆红素、凝血酶原时间、透明质酸(HA)、Ⅲ型前胶原(PCⅢ)、IV胶原(IV C)及层黏蛋白(LN)。结果:随着临床肝硬化程度的加重,血清肝纤维化指标逐渐增高,ALP呈下降趋势。血清ALP与PCⅢ、IV C及HA呈显著正相关(r=0 .2 993,P <0 .0 2 5 ;r=0 .5 5 90 ,P <0 .0 0 0 5 ;r=0 .4 4 81,P <0 .0 0 0 5 ) ,在Child pugh分级B级患者中与LN也呈明显正相关(r=0 .775 1,P <0 .0 0 0 5 )。结论:血清ALP升高与Child pugh及肝纤维化有明显关系,联合检测ALP与肝纤维化标志物对判断肝硬化患者病情及预后有重要参考意义。

Relationship among serum ALP marks of Child-pugh in patients with cirrhosis

Objective:To study the relationship among serum ALP,liver fibrosis marks and Child pugh in patients with cirrhosis.Methods:The levels of serum ALP,bilirubin,prothrombin time,HA,PCⅢ,IV C and LN in 56 patients with cirrhosis were measured.Results:It is showed that the scrum HA,PCⅢ,IV C and LN increascd and ALP droppcd gradually with the degree of clinical liver cirrhosis deteriorated.Serum ALP is positively correlated to PCⅢ( P< 0.025),IV C( P< 0.0005) and HA( P< 0.0005).Conclusion:There were close relationship between serum ALP and serum marks of liver fibrosis.Measuring the concentration of serum ALP,HA,PCⅢ,IV-C and LN simultancously has utilizable and reliable clinical value in predicting the degrees and prognosis of liver cirrhosis.
