听力障碍合并注意力缺陷多动障碍患儿背景噪音下听觉康复观察

摘    要:目的探究听力障碍合并注意力缺陷多动障碍(attention deficit hyperactivity disorder,ADHD)患儿在背景噪音下接受听觉康复的可行性。方法选择2017年1月~2020年1月于我院接受治疗的80例听力障碍合并注意力缺陷多动障碍患儿,按治疗方式分为实验组与对照组各40例,两组患儿均接受听力障碍合并注意力缺陷多动障碍干预,实验组在有背景噪音下,对照组在无背景噪音下分别进行听觉康复干预,干预6个月后对比两组患儿注意缺陷多动障碍评定量表(version IV of swanson nolan and pelham,SNAP-IV)、视听整合持续测试(integrated visual and auditory continuous performance test,IVA-CPT)、听觉行为分级(CAP)及言语可懂度分级(SIR)量表评分。结果干预前两组患儿的SNAP-IV、IVA-CPT、CAP、SIR量表得分无显著性差异(P>0.05),干预后实验组患儿的SNAP-IV、IVA-CPT量表得分显著低于对照组(P<0.05),CAP及SIR分级显著高于对照组(P<0.05)。结论听力障碍合并注意力缺陷多动障碍患儿在背景噪音下听觉言语理解能力得到较大改善。

Abstract:Objective To explore the feasibility of hearing rehabilitation in children with hearing impairment complicated with attention deficit hyperactivity disorder in the presence of background noise.Methods 80 children with hearing impairment and attention deficit hyperactivity disorder who were treated in our hospital from January 2017 to January 2020 were selected as the research objects,and were divided into experimental group and control group according to the difference in their choice of treatment methods(40 children in each group).Two groups of children received conventional hearing impairment combined with attention deficit hyperactivity disorder intervention,children in the experimental group were given auditory rehabilitation intervention under background noise,and the control group was only given auditory rehabilitation intervention.Two groups were intervened for 6 months.Compared the SNAP-IV scale,integrated audiovisual continuous test(IVA-CPT),auditory behavior classification(CAP),speech intelligibility classification(SIR)scale scores at the three time nodes before intervention,at 3 months and 6 months of intervention of the two groups of children.Results Before the intervention,the scores of the SNAP-IV scale,DSM-IV scale,CAP scale and SIR scale were not significantly different between the two groups of children(P>0.05).After the intervention,the scores of SNAP-IV scale and IVA-CPT scale of the experimental group scale were significantly lower than those of the control group(P<0.05),and the CAP and SIR grades were significantly higher than those of the control group.Conclusion Children with hearing impairment combined with attention deficit hyperactivity disorder have a better effect of hearing rehabilitation training with background noise.After intervention,their speech comprehension and auditory level are both improved a lot.
