原发性肝癌的术后复发类型

介绍原发性肝癌根治术后复发28例,均经再次手术及病理证实,或术后AFP再次升高而确诊。肝癌术后复发分为切缘单结节、残肝单结节及多结节复发三种类型。术后第一年复发18例,占总复发的64.3％,切缘单结节及多结节复发多发生在第一年内。本文提出了术中轻柔操作减少挤压、术前综合治疗使肿瘤局限或缩小、术中仔细探索不遗留肿瘤组织、术后合理应用化疗、免疫、放疗等四项预防肝癌术后复发的措施。

PATTERNS OF INTRAHEPATIC RECURRENCE AFTER CURATIVE RESECTION OF HEP A TOCELLULAR CARCINOMA

28 cases of intrahepatic recurrent patients after curative resection of hepatoceilular carcinoma were reviewed. The diagnosis were made by pathologic examination of AFP determination of the blood. The recurrent patterns included the singular nodule near the incision of the liver, the siongular nodule in the remainder liver tissue, and multiple nodules. 18 cases recurred in the first year after operation, The recurrence can be prevent by pre-operative treatment, intra-operative manipula tion, curative resection and the post-operative therapies.
