肝动脉化疗栓塞术联合经皮微波消融治疗原发性肝癌临床研究

目的探讨肝动脉化疗栓塞术（TACE）联合经皮微波消融（PMCT）治疗原发性肝癌的疗效。方法73例原发性肝癌患者随机分为联合治疗组和单纯TACE治疗组；联合治疗组32例共46个结节，病灶直径平均（4．9±0．5）cm；单纯TACE组41例共51个结节，病灶直径平均（5．1±0．6）cm。单纯TACE治疗组仅行TACE治疗，联合治疗组在TACE术后1～2周行PMCT。所有患者术后1个月行增强CT扫描评价治疗效果。结果联合治疗组瘤体完全坏死率86％，单纯治疗组瘤体完全坏死率57％，2组间差异有统计学意义（P〈0．05）。结论TA—CE术联合PMCT治疗原发性肝癌疗效优于单纯TACE术。

The clinical study of TACE combined with PMCT in treatment of primary hepatic carcinoma

Objective To investigate the therapeutic effect of transcatheter arterial chemoembolization （TACE） combined with percutaneous microwave coagulation therapy （PMCT） for primary hepatic carcinoma （PHC）. Methods 73 patients of PHC were divided into combination therapy group （ n =32） and TACE alone treatment group （ n = 41 ）. All of the patients were treated with TACE, while the PMCT was implemented in 1 - 2 weeks after TACE for combination therapy group. 1 month after treatment, all patients received enhancement CT scans to evaluate the therapeutic effect. Results Complete tumor necrosis rate was 86% in combination therapy group and 57% in TACE alone treatment group. The difference between the 2 groups was statistically significant （ P 〈 0.05 ）. Conclusion The therapeutic effect of TACE combined with PMCT for PHC is better than TACE alone.
