临床各型颈椎病影像诊断对照研究

目的 研究临床各型颈椎病的X线、CT、MRI的影像学表现及特点,探讨各型颈椎病的最有效的影像学检查方法.方法 对80例颈椎病患者进行X线、CT和MRI检查,并对检查结果 进行分析.结果 神经根型颈椎病主要影像学表现为颈椎退行性改变、椎间盘突出、神经根受压等征象;脊髓型颈椎病主要表现为颈椎退行性改变、椎间盘突出、脊髓受压、椎管狭窄等征象;椎动脉型颈椎病主要表现为颈椎退行性改变以及CTA或MRA显示椎动脉扭曲、狭窄、变形等征象;而交感神经型颈椎病主要表现为颈椎退行性改变,但没有其他典型征象,影像诊断尤其困难,主要依赖于临床诊断.结论 影像学检查对临床各型颈椎病的鉴别诊断起到至关重要的作用,合理选择影像学检查方法,可以经济而有效地达到诊断临床各型颈椎病的目的.

Comparative study on clinical imaging diagnosis of cervical diseases

Objective To study the imaging of various types of clinical cervial diseases, and discover the most effective imaging method. Methods 80 patients suffering from cervical syndrome were examined by radiography, computed tomography and magnetic resonance. The diagnostic results were comparatively analyzed. Results Cervical degenerative changes, intervertebral disk hernia, nerve root compression were the main imaging of nerve root type cervical spondilosis ; Cervical myelopathy＇s manifestations were cervical degenerative changes, interverbral disk hernia, spinal cord compression, and spinal stenosis, etc. The imaging of vertebral artry type of cervical disease included that cervical degenerative changes, and normal or thinning, twisting localized stenosis and course anormaly of vertebral artery, etc. The main imaging of the cervical sympathetic was the cervical degenerative changes, because of no other typical signs, it＇s particularly difficuh for diagnostic imaging, it＇s mainly based on clinical dignosis. Conclusion Imaging for various clinical differential diagnosis of cervical disease plays a crucial role. Reasonable choice of imaging methods can achieve the most economical and effective diagnostic purpose.
