| 摘 要： | 目的探讨老年低位直肠癌患者行保肛根治术的临床应用。方法回顾和分析1993年8月～2003年7月10年间在我院行保肛根治术的56例年龄超过65周岁的低位直肠癌患者的临床资料，分析吻合口瘘的发生率及发生原因，并与同时期74例行经典Miles术的相同年龄段的低位直肠癌患者比较局部复发率以及1、3、5年生存率。结果保肛根治术局部复发率为15．3％，1年、3年和5年存活率分别为96．4％、69．6％和55．4％；而Miles术则分别为14．9％、96．0％、71．6％和52．7％，两者差异均无显著性（P〉0．05），结论老年低位直肠癌患者行保肛根治术，可获得与经典Miles术相近的生存率，局部复发率也相近，通过控制吻合口瘘等并发症的发生，可明显提高患者术后的生活质量，值得临床推广应用。
