虹膜固定型有晶状体眼人工晶状体植入术矫治近视的临床观察

目的 探讨高度近视眼行虹膜固定型有晶状体眼人工晶状体(IFIOL)植入术的安全性和有效性.方法 选取2005年7月至2009年2月接受IFIOL植入术治疗高度近视的患者共89例(169只眼).术前最佳矫正远视球镜屈光度数为-10.00～-23.00D,最佳矫正远视力0.1～0.8,植入IFIOL,术后随访1～36个月.观察指标包括:术前及术后裸眼及矫正视力、屈光度、眼压、瞳孔形态、角膜内皮计数、UBM观察人工晶状体的位置及眼前节的变化情况,对数据指标进行统计学分析.结果 所有病例裸眼远视力均较术前最佳矫正远视力提高1行以上,视力提高2行以上者127只眼占75.15%;术后观察指标无明显异常.14只眼(8.28%)诉有眩光,5只眼(2.96%)瞳孔轻度变形,7只眼(4.14%)术后两天切口渗漏,4只眼(2.37%)人工晶状体襻松脱,1只眼(0.59%)1年后出现白内障.无视网膜脱离、眼内炎、葡萄膜炎等其他并发症.结论 虹膜固定型有晶状体眼人工晶状体植入术治疗高度近视安全、有效,并且具有良好的预测性,远期效果及相关并发症需进一步观察.

Clinical Observation of Iris-fixated intraocular lenses for Treating 169 High Myopia

Objective To evaluate the efficiency and safety of iris-fixated intraocular lenses (IFIOL) for treating high myopia.Methods In this study, 89 cases (169 eyes) with high myopia at-10～-23.0 diopters (D) were implanted with iris-supported PIOL, the best corrected visual acuity (BCVA) was between 0.1～0.8.The following observation was longer than 12 months in all cases.Uncorrected visual acuity (UCVA), best spectacled-corrected visual acuity (BSCVA), refraction, intraocular pressure (IOP), cornea topography,pupil, corneal endothelial count, IOL position, complication of all the treated eyes were observed.All data were analyzed in statistics.Results All of the cases had gained 1 line of their preoperative BSCVA, 75.15% (127 eyes) gained 2 or more lines; the intraocular pressure and corneal endothelial cell density were not statisti-cally significant difference between pre and post operative.Of 14 eyes (8.28%) complained of halo and glare especially at night, 5 eyes (2.96%) pupils were slightly distorted, 7 eyes (4.14%) had seepage for two days after operation, 4 eyes (2.37%) had IOL dislocation, 1 eye had cataract one year later.There were no retinal detachment, uveitis or endophthalmitis occurred in our study.Conclusions Iris-fixated intraocular lenses (IFI-OL) for treating high myopia are effective, safe and predictable.The effectiveness and safety of the PIOL should be further observed in China.
