原发性高血压患者骨桥蛋白与醛固酮、颈动脉内膜中层厚度的相关性研究

摘    要:目的 探讨原发性高血压(EH)患者血浆骨桥蛋白(OPN)、血清醛固酮(ALD)水平与颈动脉内膜中层厚度(IMT)的关系.方法 选择EH患者91例,按IMT值分为EH无颈动脉硬化组50例和EH合并颈动脉硬化组41例.对照组为同期常规健康体检正常者39例.测定血浆OPN、血清ALD水平.同时做超声测量颈动脉IMT值.结果EH无颈动脉硬化组较对照组血浆OPN(31.56±4.06)μg/L vs (29.54±5.64) μg/L、血清ALD( 134.61±19.23)ng/L vs(125.04±21.77)ng/L水平升高(均P<0.05);EH合并颈动脉硬化组较对照组升高更显著:血浆OPN(34.20±4.35)μg/L vs(29.54±5.64)μg/L、血清ALD(143.95±24.07) ng/L vs(125.04±21.77)ng/L(均P<0.01).EH患者血浆OPN与血清ALD水平(r=0.594,P<0.05)及颈动脉IMT(r=0.424,P<0.01)存在正相关.结论EH患者血浆OPN水平升高,且与血清ALD水平及颈动脉IMT存在正相关;提示EH患者血浆OPN升高可能在其动脉粥样硬化发生发展中的作用.

Relationship between plasma osteopontin, serum aldosterone and carotid intima media thickness in essential hypertensive patients

Abstract:Objective To determine plasma osteopontin and serum aldosterone levels in patients with essential hypertension and investigate the relationship between plasma osteopontin level and carotid atherosclerosis. Methods Plasma osteopontin and serum aldosterone levels were determined in 91 essential hypertensive patients and 39 healthy controls,respectively.Carotid intima media thickness(IMT) was measured with color Doppler ultrasound.The patients were divided into group without carotid artery atherosclerosis(50 cases) and group with carotid artery atherosclerosis(41 cases),depending on IMT.39 healthy subjects served as controls depending on routine health examination. Results Plasma osteopontin(31.56±4.06) μg/L vs(29.54±5.64) μg/L and serum aldosterone(134.61±19.23) ng/L vs(125.04±21.77) ng/L(both P<0.05) levels were increased in 50 essential hypertensive patients with IMT<0.9 mm as compared with 39 healthy controls.Plasma osteopontin(34.20±4.35) μg/L vs(29.54±5.64)μg/L and serum aldosterone(143.95±24.07) ng/L vs(125.04±21.77) ng/L(both P<0.01) levels were increased more significantly in 41 essential hypertensive patients with IMT≥0.9 mm as compared with 39 healthy controls.There was positive relationship between plasma osteopontin and serum aldosterone level in essential hypertensive patients(r=0.594,P<0.05).A positive relationship was found between IMT and plasma osteoponin(r=0.424,P<0.01) level in essential hypertensive patients. Conclusion Plasma osteoponin level increases in essential hypertensive patients,especially in the patients with carotid atherosclerosis.Plasma osteoponin level is positively correlated with carotid IMT and plasma aldosterone level,suggesting the role of osteoponin in process of atherosclerosis.
