辅助性T细胞17及其分泌的白细胞介素17在类风湿性关节炎患者的表达

目的 通过检测辅助性T细胞17(Th17)及其分泌的白细胞介素17(1L-17)在类风湿性关节炎(RA)患者的表达,探讨其在RA发病机制中的作用及其临床意义.方法 应用流式细胞术检测39例RA患者和20例健康对照者外周血Th17的比率,同时用酶联免疫吸附测定(ELISA)法检测IL-17的水平.结果 RA患者外周血Th17比率和血清中IL-17的水平均较健康对照组显著增高,(1.86±0,75)％ vs (0.46±0.73)％,(13.42±0.62) ng/L vs(5.38±1.67)ng/L (P＜0.01).RA未治疗组外周血Th17比率和血清中IL-17的水平显著高于治疗后未复发组,(2.42±0.69)％ vs (0.84±0.62)％,(14.63±6.02) ng/L vs (8.86±1.35) ng/L(P＜0.01),但未治疗组与治疗后复发组比较差异无统计学意义(P＞0.05)；RA患者关节液中IL-17水平明显高于血清中的水平(13.76±0.49)ng/Lvs (12.38±0.54) ng/L( P＜0.05).结论 RA患者Th17的比率及其IL-17的水平均高,它们可能参与了RA的发病和病情进展.

Expression of Th17 cells and its secretive interleukin-17 in patients with rheumatoid arthritis

Objective To detect the levels of Th17 cells and its secretive interleukin 17（IL-17） in patients with rheumatoid arthritis（RA）,and to investigate their effects in the pathogenesis of RA and clinical significance.Methods Thirty-nine RA patients and 20 healthy subjects were included in the study.Flow cytometric assay was used to detect the rate of Th17 cells in the peripheral blood of RA patients and healthy subjects.Simultaneously,ELISA was used to detect the level of IL-17.Results The rate of Th17 cells and the level of serum IL-17 significantly increased in RA patients as compared with those in healthy subjects,（1.86±0.75）% vs（0.46±0.73）%,（13.42±0.62） ng/L vs（5.38±1.67） ng/L（P0.01）.Untreated cases group was significantly higher than clinical recovered cases in the rate of Th17 and the level of serum IL-17,（2.42±0.69）% vs（0.84±0.62）%,（14.63±6.02） ng/L vs（8.86±1.35） ng/L（P0.01）,but there were no significant differences in the above two indexes between the untreated patients and the recurrent patients（P0.05）.IL-17 level in joint fluid significantly increased than that in serum,（13.76±0.49） ng/L vs（12.38±0.54） ng/L（P0.05）.Conclusion The rate of Th17 cells and the level of serum IL-17 significantly elevate in RA patients.They might be involved in the activation and development of RA.
