腔镜辅助手术治疗下肢原发性静脉曲张

目的:探讨腔镜技术在下肢原发性静脉曲张手术中应用的可行性和结果。方法:腔镜辅助手术治疗24例(32条患肢)下肢原发性静脉曲张病人。术后随访3～22个月;传统手术治疗20例(27条患肢)下肢原发性静脉曲张病人.术后随访5～24个月。结果:腔镜辅助手术治疗24例病人术后肢体症状和浅静脉曲张消失,16例合并有静脉性溃疡患者短期内溃疡愈合,随访3～22个月只有1(1/32)条患肢复发静脉曲张,其余病人未见静脉曲张复发及并发症;而传统手术治疗20例病人术后肢体症状和浅静脉曲张消失,10例合并有静脉性溃疡患者短期内溃疡愈合,随访5～24个月有3(3/27)条患肢复发静脉曲张,2(2/10)条患肢复发溃疡。结论:腔镜辅助手术治疗下肢原发性静脉曲张手术效果好,术后复发率低,患肢创伤小,病人满意度高,是治疗下肢原发性静脉曲张的有效方法。

Endoscope-Assisted Correction of Primary Varicose Veins in the Lower Limb

Objective: This article is to evaluate the possibilities and results of endoscopic surgery in the operation of primary varicose veins of the leg. Materials and Methods: Primary varicose veins in 32 limbs in 24 patients were operated with the assistance of endoscopic surgery. All 24 patients were reviewed 3 to 22 months postoperatively. And primary varicose veins in 27 limbs in 20 patients were operated with conventional surgery. All 20 patients were reviewed 5 to 24 months postoperatively. Results: Endoscope -assisted surgery obviously improved venous disbility , varicose vein disappeared; ulcers healed shortly in 16 cases. Only one limb had recurrent varices at a new site during follow - up 3 to 22 months postoperatively. Ninety - seven percent of limbs (31of32) had no recurrence of varicose veins and no complication. Conventional surgery obviously improved venous disbility and varicose vein disappeared in all patients and ulcers healed shortly in 10 cases, but three limb had recurrent varices and two limb had recurrent venous ulcers during follow - up 5 to 24 months postoperatively. Conclusion: In this series , the recurrent rate varicose veins was low and postoperative scarring was minimized, in addition to advantages of endoscopic surgery. These results demonstrate that endoscopic surgery is a worthy alternative procedure for correcting primary varicose veins.
