50例乳腺浸润性导管癌的超声诊断分析

目的研究乳腺浸润性导管癌（iDC）的超声声像图特征，明确超声诊断此病的临床应用价值。以提高超声诊断水平。方法对50例经手术、病理证实为乳腺浸润性导管癌患者的临床资料以及乳腺超声声像图进行回顾性分析，总结其超声声像图特征。结果50例患者的声像图特征可以分为三类：29例为团块型，20例患者为结节型，1例患者为囊实型：26例患者肿块形态不规则，其中18例患者为圆形，8例患者为卵圆形。伴有钙化及丰富的血流信号为浸润性导管癌的特征性表现。内部回声：49例患者有低回声，1例患者有等回声；钙化表现：39例患者有钙化表征出现，11例患者无钙化征象；后方伴回声衰减19例。不变11例；20例患者有淋巴结转移；40例患者肿块内有血流信号。结论乳腺浸润性导管癌在超声声像图上具有较明显的特征，超声可以成为诊断乳腺浸润性导管癌的有效的影像学方法，能够提高乳腺浸润性导管癌的诊断符合率，对临床医师制定完善的治疗方案具有重要意义。

Ultrasonic Diagnosis of 50 Cases of Breast Invasive Ductal Carcinoma

Objective To study the ultrasonographic features of breast invasive ductal carcinoma （IDC）, identify the clinical application value of ultrasonic diagnosis and improve the level of diagnostic ultrasound. Methods The clinical data and ultrasonic image of patients with breast invasive ductal carcinoma confirmed by operation and pathology were analyzed retrospectively to summarize the ultrasonographic features. Results The ultrasonographic features of 50 patients could be divided into 3 categories： 29 cases of mass type, 20 cases of nodular type and 1 case of cystic and solid type. 26 cases had irregular lumps with 18 cases of circular and 8 cases of oval. Calcification and abundant blood flow signal were the characteristic fmdings oflDC. 49 cases had low level echo and 1 case had equal echo. Calcification characterization was demonstrated in 39 patients and 11 patients had no calcification sign. The rear with echo attenuation appeared in 19 patients, unchanged in 11 cases. 20 cases had lymph node metastasis and 40 patients had blood flow signal in the mass. Conclusions Breast invasive ductal carcinoma has obvious ultrasonographic characteristics. Ultrasound can be the effective imaging method in the diagnosis of breast infiltrative ductal carcinoma, and improve the diagnosis coincidence rate, which has important significance for the clinicians to establish perfect treatment options.
