脑梗死女性患者颈部动脉粥样硬化与雌二醇水平的相关性研究

摘    要:目的探讨脑梗死女性患者颈部动脉粥样硬化与雌二醇水平的关系。方法选取2013年2月至2014年2月我院收治的90例绝经后脑梗死女性患者,采用彩色多普勒超声对入选患者进行颈部动脉检查,观察颈部动脉粥样硬化情况,按照有无粥样硬化斑块分为斑块组60例(其中轻度26例,中度20例,重度14例)和非斑块组30例。检测所有入选患者血清雌二醇(E2)和睾酮(T)、胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C)和纤维蛋白原(FI)水平,分析E2水平与动脉粥样硬化程度之间的关系。结果斑块组患者血清E2及E2/T水平明显低于非斑块组,在斑块组的各亚型中重度组最低,中度组次之,轻度组最高,差异具有统计学意义(P〈0.05);斑块组患者血清T水平明显高于非斑块组,在斑块组的各亚型中重度组最高,中度组次之,轻度组最低,差异具有统计学意义(P〈0.05);斑块组患者血清HDL-C水平明显低于非斑块组;TC、TG、LDL-C及FIB水平明显高于非斑块组,差异具有统计学意义(P〈0.05)。Pearson相关性分析显示,绝经后脑梗死女性患者的E2水平与动脉粥样硬化程度存在线性关系,呈负相关(r=0.76,P〈0.05)。结论雌二醇水平下降与绝经后脑梗死女性患者颈部动脉粥样硬化斑块形成关系密切,机制可能与雌二醇调节血脂水平有关。

Abstract:Objective To discuss the correlation between estradiol level and carotid atherosclerosis in females with cerebral infarction. Methods Ninety postmenopausal females with cerebral infarction(hospitalized from February 2013 to February 2014) from the hospital were selected and observed for their carotid atherosclerosis conditions by adopting the color Doppler ultrasound examination in the carotid artery. According to the presence or absence of atherosclerotic plaques, the subjects were divided into two groups: the plaque group(totally 60 cases, with 26 mild cases, 20 moderate cases and 14 severe cases) and the non-plaque group(totally 30 cases). All subjects' serum estradiol(E2) and testosterone(T), cholesterol(TC), triglyceride(TG), low density lipoprotein(LDL-C), high-density lipoprotein(HDL-C) and fibrinogen(FIB) level were tested, and then analysis of the correlation between estradiol level and atherosclerosis level was performed. Results The level of serum E2 and E2/T of the plaque group were significantly lower than that of the non-plaque group. As for each subtype in the plaque group: the level of severe group was the lowest, followed by the medium group, and level of the mild group was the highest. And the difference has reached a level of statistical significance(P〈0.05); the serum T level of the plaque group was significantly higher than that of the non-plaque group.
