扩大前颅底入路显微手术切除前颅底鼻窦沟通瘤

目的报告经扩大前颅底入路切除前颅底鼻窦沟通瘤的手术效果。方法回顾性分析35例前颅底鼻窦沟通瘤的临床特征、手术方法及疗效。结果26例肿瘤全切，9例次全切。术后15例行放射治疗。随访0．5～9年，5例恶性肿瘤患者死亡，30例健在。结论扩大前颅底手术入路的优点是肿瘤全切率高，显露广，不易损伤周围正常组织，不影响面容，便于颅底重建。

Microsurgical treatment of tumors of anterior skull base by extended anterior subcranial approach

Objective To report the clinical results of microsurgical treatment of anterior skull base tumors by extended anterior subcranial approach. Methods Thirty patients with anterior skull base tumors which involved the paranasal sinuses were performed operation via the extended anterior subcranial approach. Results Total resection was achieved in 26 patients and subtotal resection in 9 patients. Fifteen patients were performed radiotherapy. All patients were followed up from 0. 5 to 9 years. Of 20 malignant tumors, only 5 died of tumor recurrence after operation. Conclusion The extended anterior subcranial approach has the advantages of scar-free incision on the face, wide exposure of all anterior skull base planes, no lesion to the neighboring structures and it is also helpful for the reconstruction of defects on the skull base.
