肝脏炎性假瘤的超声和CT对比研究

摘    要:目的对比研究肝脏炎性假瘤的超声和CT诊断价值。方法对经手术及病理证实的12例共14个肝脏炎性假瘤进行了B超、CT和彩色多普勒超声(CD)的对比分析。结果在B超上,肝脏炎性假瘤可表现为低同声不规则形病灶,后方无增强等改变;CT上可为低密度不规则形.增强后强化不明显,动态扫描无快进快出等现象,CD显示多为无明确彩色血流。在本组中,B超、CT和CD诊断为良性病变的符合率为21.4%、36.4%及88.9%;诊断为肝脏炎性假瘤的符合率分别为7.1%、9.1%及66.7%。结论对肝脏炎性假瘤的诊断应首先加强认识。如B超出现不规则形低回声病灶应作CD检查,而CT发现无明显增强的不规则形低密度灶,应加作动态检查。这样有利于提高诊断的正确性。

Abstract:Objective To evaluate CT and ustrasoand in diagnosis of znflammatory pseudotamor of the liver. Methods Analysis of 14 lesons with infpl-mmatory pseadotumor of the Liver(IPL) was carried out by vsing ultrasonographycus), CT and color Doppler ultrasoand(CD). Results The manifestations of IPL in us were hypoechoie lesion with inhomogeneity, irregularty shaped calabashlike mass without posterior acoustic enhancement, while on CT scan, an irrenularly shaped lowdensity leson which did not enhance after contrast injection was seen. The blood supply in most lesions was not observed on CD imaging. The accurate rates of us, CT and CD for the diaghosis of benign tumor in our group were 21.2%, 36.4% and 88.9% respectively; and the accurate rates for the cliagnosis of IPL were 7.1%, 9.1% and 66.7% respectively. Conclusions The accaracy of US CD was much higher than that of CT or us alone. Therefore, CD can be applied when a hypoechoic lesion with irregular shape is displayed on us. If no enhancement is found ofter contrast stady, then dynamic CT is recommended.
