椎管内囊肿的诊断与治疗

目的 为了提高对椎管内囊肿的诊断及治疗水平。方法 回顾分析本组１２例患者，临床表现以肢体麻木，疼痛，力弱、二便障碍为多见，ＣＴ及ＭＲＩ有助于确诊。根据囊肿部位，临床分为客内硬脊膜外囊肿，髓外硬脊膜下囊肿及神经根袖套部囊肿（Ｔａｒｌｏｖ囊肿）。结果 １２例均行手术治疗，随访表明８例症状消失，３例好转，１例症状无改善。结论 对椎管内囊肿症状及体征明显者，应施行手术治疗。手术要点为切除囊肿壁及蒂部，同时

DIAGNOSIS AND SURGICAL TREATMENT OF INTRASPINAL CYSTS

Objective To improve the diagnosis and treatment level of intraspinal cysts. Methods 12 cases of intraspinal cysts were analysed retrospectively. Back pain, extremity numbness and weakness or bowel and bladder disturbances were the common complaints made by the patients. CT and MRI scans were helpful to the diagnosis. Intraspinal cysts, according to their location, were divided into 3 types: extradural, intradural and perineurial (Tarlov) cysts. Results All the patients received operation. Follow-up study showed that 8 cases were restored, 3 turned better and 1 remained unimproved. Conclusion Operative treatmetn is necessary for the patients with progressive symptoms and signs. The key points of operation lie in the resecting of the cyst wall and neck in order to release the compression on the nerve roots and spinal cord.
