甲状腺未分化癌的诊治（附2例报道）

回顾苏州大学附属第一医院甲乳科2016年及2017年收治的2例甲状腺未分化癌的临床资料，结合文献复习。2例患者术前胸部CT均提示有明显肺转移，行姑息性手术及颈部放化疗后，病情仍进展迅速，再次出现肿瘤，均于4个月内死亡。提示甲状腺未分化癌高度恶性，易侵犯气管、食管以及喉返神经等相邻组织，预后差。应手术、放化疗治疗，同时结合患者全身状况及患者意愿综合考虑。阿帕替尼作为一种靶向药物可用于甲状腺未分化癌的治疗。

Combining analysis of two cases of anaplastic thyroid cancer in the first affiliated hospital of Soochow university,explore the best treatment for anaplastic thyroid cancer

【Abstract】Objective To explore the best treatment for anaplastic thyroid cancer.Methods Reviewing some related literatures，combining analysis of two cases of ananplastic thyroid cancer in the department of thyroid and breast surgery in the first affiliated hospital of Soochow university.Results In both cases，the preoperative chest CT showed obvious pulmonary metastasis，the condition progressed rapidly，palliative surgery，neck radiotherapy and chemotherapy were performed in both patients，the prognosis of patient Jin was very poor，patient Mei's shoulder mass was rapidly developed after the operation.Conclusion anaplastic thyroid cancer develops rapidly，easily invades adjacent structures such as trachea，esophagus，and recurrent laryngeal nerve，the prognosis is very poor，the treatment of ananplastic thyroid cancer should be multidisciplinary and comprehensive，the clinical stage should be based on the preoperative examination report，then decide the specific treatment，the treatment should be a combination of surgery，radiotherapy and chemotherapy.
