阴道彩色多普勒超声诊断子宫瘢痕处早期妊娠的价值及临床分析

目的 探讨经阴道彩色多普勒超声诊断子宫瘢痕处早期妊娠的价值。方法 选择本院2012年1月~2013年12月收治的60例子宫瘢痕处早期妊娠患者作为研究对象,随机分为A组和B组,各30例,A组行腹部彩超,B组行经阴道彩色多普勒超声,分析超声诊断的图像特点。结果 A组检出子宫瘢痕处早期妊娠21例（70.0%）,误诊为早孕流产4例、宫颈妊娠2例、宫内妊娠3例;B组检出子宫瘢痕处早期妊娠28例（93.3%）,误诊为滋养细胞肿瘤1例、宫颈妊娠1例。B组检出效果显著优于A组,差异有统计学意义（P〈0.05）。根据超声诊断,可将子宫瘢痕处早期妊娠划分为孕囊型、不均质包块型和混合型三类。结论 经阴道彩色多普勒超声可有效诊断子宫瘢痕处早期妊娠,值得临床推广应用。

Clinical analysis and value of transvaginal color doppler ultrasound in the diagnosis of uterine scar in early pregnancy

Objective To exsplore the value of transvaginal color doppler ultrasound in the diagnosis of uterine scar in early pregnancy. Methods 60 patients with uterine scar in early pregnancy from January 2012 to December 2013 in our hospital were selected and randomly divided into A group and B group,30 cases in each group.Underwent abdominal ultrasound was used in A group,and transvaginal color doppler ultrasound was used in B group.The image characteris- tics of ultrasonic diagnosis were analyzed. Results The A group was detected in 21 cases （70.0%） of uterine scar preg- nancy,4 cases were misdiagnosed as early pregnancy abortion,2 cases were misdiagnosed as cervical pregnancy,1 case was misdiagnosed as intrauterine pregnancy;The B group was detected in 28 cases （93.3%） of uterine scarpregnancy,1 case was misdiagnosed as trophoblastic tumor,1 cases was misdiagnosed as cervical pregnancy.The detecting effect of B group was significant better than that of A group,with statistical difference（P〈0.05）.By ultrasound,uterine scar in early pregnancy was divided into the gestational sac type,heterogeneous mass type and mixed type. Conclusion Transvaginal color doppler ultrasound can effectively diagnose uterine scar in early pregnancy,and it is worthy of clinical promotion and application.
