超声STI评价2型糖尿病患者左室心肌旋转及扭转的研究

目的 应用超声STI探讨并评价2型糖尿病患者左室心肌旋转与扭转的特点。方法 选取2012年1月~2014年1月本院诊治的2型糖尿病患者60例作为研究组,依据临床是否合并微血管病变分为两组,30例单纯糖尿病患者作研究A组,30例糖尿病微血管病变患者作研究B组;选取30例同期在本院检查结果示血糖正常、无心脏病的健康体检者作为对照组,分析三组的常规超声心动图指标与STI指标变化情况。结果 三组的LVEF指标组间比较,差异无统计学意义（P〉0.05）;研究B组的E/A、Peaktw与UntwR指标显著低于研究A组,差异有统计学意义（P〈0.05）;与对照组比较,研究A、B两组的LVPWT、IVST、EN-PAR、EP-PAR与UHT均显著上升,E/A、Peaktw、AVCtw、MVOtw、IUR与UntwR水平均显著下降,差异有统计学意义（P〈0.05）。结论 2型糖尿病患者左室心肌旋转与扭转的STI指标有明显特点,可反映患者心肌病变。

Research on ultrasonic STI evaluation of left ventricular rotation and torsion in patients with 2-type diabetes

Objective To explore and evaluate the characteristics of left ventricular rotation and torsion by speckle tracking imaging in patients with type 2 diabetes. Methods 60 cases with 2-type diabetes from January 2012 to Jan uary 2014 in our hospital were selected and divided into two groups according to microvascular lesions,the A research group for 30 cases with single diabetes and the B research group for 30 cases with microvascular lesions of diabetes.30 cases over the same period in our hospital blood test results show normal,healthy people without heart disease were selected as a control group.The indexes of conventional echocardiogram and STI of three groups were analyzed. Results There was no statistical significance in the indexes of LVEF between three groups （P〉0.05）.E/A,Peaktw and UntwR of the B research group was lower than that of the A research group,with statistical difference （P〈0.05）.Compared with the control group,LVPWT,IVST,EN-PAR,EP-PAR and UHT of the A research group and the B research group were signifi- cantly increased,and E/A,Peaktw,AVCtw,MVOtw,IUR and UntwR were significantly reduced,with statistical difference （P〈0.05）. Conclusion STI indexes of left ventricular rotation and torsion in patients with 2-type diabetes have evident characteristics,and can reflect myocardial lesions.
