腰椎棘突间内固定器的基础研究及临床应用★

摘    要:腰椎棘突间内固定器是脊柱外科后路非融合置入物的一类,根据其特性及作用特点可分为静态系统和动态系统两类。随着内固定器制作工艺的飞速发展,腰椎棘突间内固定器的材料构成范围也很广泛,包括同种异体骨移植物、钛、聚醚醚酮和人造橡胶复合物等。大量的基础及临床研究表明,腰椎棘突间内固定器在退变性椎管狭窄、椎间盘原性下腰痛、关节突综合征、椎间盘突出症和腰椎不稳等疾病的治疗中有着广阔的应用前景,但其中也有许多问题,诸如腰椎棘突间内固定器对腰椎后柱结构的影响及其对腰椎间盘的作用机制和对脊柱稳定性的影响等将有待于进一步解决,其临床适应证也有待于长期随访最终确证。

Abstract:Interspinous process device is a kind of non-fusion implant for posterior spinal surgery. It is classified by static system and dynamic system according to its characteristics. With rapid development of internal fixator manufacture, materials for interspinous process device become increasing, such as bone allograft, titanium, poly(ether-ether-ketone) and lactoprene composite. A large number of experimental and clinical researches reveal the advantages of this technology in degenerative spinal stenosis, intervertebral original lower back pain, facet syndrome, protrusion of intervertebral disk, and lumbar instability. However, there are still many problems such as the influence on the lumbar posterior column, the mechanism to lumbar disc and the effect on the stability of spine etc. In addition, the clinical indications need to validate by long-term follow up.
