脑干听觉诱发电位在脑出血微创术前后的改变

摘    要:目的 观察高血压脑出血患者微创术前、后脑干听觉诱发电位 (BAEP)的变化 ,辅助评价微创术的疗效。方法 对 36例高血压脑出血患者进行微创术治疗 ,监测术前、术后 2 4,48,72h以及术后第 7天的BAEP改变。结果 本组存活 30例 ,6例死亡 ,死亡率为 16.7%。术前BAEP异常主要表现为潜伏期 (PL)及峰间潜伏期 (IPL)延长 ,与正常对照组相比 ,差异有显著性 (P <0 .0 5 )。术后 2 4h内复查 ,PL和IPL均有明显下降 ,与术前相比 ,差异有显著性意义 (P <0 .0 5 )。术后 48,72h和 7d复查BAEP ,与术前相比 ,PL和IPL虽然下降 ,但差异无明显统计学意义 (P >0 .0 5 ) ;与正常对照组相比 ,PL和IPL仍有延长 (P <0 .0 5 )。结论 微创术安全、有效。BAEP作为急性缺血、缺氧时脑干功能状态的一项检测指标可以用于高血压脑出血患者微创术前、后监测及预后评估

Abstract:Objective To evaluate the efficacy and safety of minimally invasive surgical intervention in treating the patients of hypertensive intracerebral hemorrhage (ICH) by monitoring the changes of brainstem auditory evoked potential (BAEP). Methods The minimally invasive surgical intervention was applied for the treatment of 36 patients with ICH. Serial measurements (pre-operation, and 24h, 48h, 72h and 7days post-operation) of the BAEP were made. Results The mortality was 16.7% (6/36). Before operation, both of the peak latency(PL) and the interpeak latency(IPL) in BAEP were significantly prolonged as compared to those of the normal controls(P< 0.05 ). Twenty-four hours after the operation, the PL and IPL were shortened sharply in comparing with the pre-operative state(P<0.05).But no significant difference was revealed between the pre-operation period and 48h,72h and 7days after the operation as regard to the PL and IPL. However, compared to the normal controls, the PL and IPL were still prolonged (P<0.05). Conclusion BAEP can be used as one of the indices to monitor the brainstem function in the patients with ICH before and after the minimally invasive surgery.
