三联蛋白芯片法在胰岛自身抗体检测中的临床应用

目的:评价三联蛋白芯片检测系统在胰岛自身抗体(GAD-A、ICA、IAA)检测中的临床应用价值。方法:采用国内三联蛋白芯片法测定51例糖尿病病人胰岛自身抗体的结果与酶联免疫法(ELISA)测定的GAD-A,ICA,放免法(RIA)测定IAA结果进行比较。结果:蛋白芯片法阳性检出率GAD-A23·5%,ICA7·8%,IAA27·4%;酶联免疫法检测阳性GAD-A33·3%,ICA41·2%,放免法IAA19·6%。上述两组结果进行比较,两组相比ICA有统计差异(P<0·05),GDA-A、IAA无统计差异。两组结果一致率比较GAD-A62%;ICA64·7%;IAA64·7%。结论:与临床常用ELISA法检测GAD-A,ICA,放免法检测IAA比较,三联蛋白芯片法和ELISA法在ICA阳性检出率有显著差异。

Clinical Application of Combined Proteinchip Array in Detection for Islet Autoantibodies

Objective:To evaluate the assay of combined protein chip in detection for Islet Autoantibodies:glutamic acid decarboxylase antibody(GAD-A),islet cell antibody(ICA) and insulin autoantibody(IAA).Methods:The blood serum of 51 patients with diabeties mellitus were measured GAD-A and ICA with ELISA assay while IAA with radioimmunoassay(RIA).The same sample is detected by combined protein chip too.Results:The positive rates of GADA,ICA,IAA are 33.3%,41.2%,19.6% with traditional assays and 23.5%,7.8%,27.4% with combined proteinchip array.The antibodies positive rates of GADA and IAA were no difference between two groups(P>0.05) while ICA were significantly higher by ELISA(P<0.05).The concordance rates of two groups are 62%(GADA),64.7%(ICA),64.7%(IAA).Conclusion:The two different assays have different sensitivity and specificity.Combined proteinchip is much quicker and agiler than traditional ways,the results have a long time stable state and can be judged by naked eyes,its matching instruments are simple.It is a convenient tool for emergency detection.Its technique is worthy of developing.
