视网膜母细胞瘤结合蛋白4 在胃癌组织中的表达水平及其临床意义

目的探讨视网膜母细胞瘤结合蛋白4(RBBP4)在胃癌组织中的表达水平、临床意义及其与预后的相关性。方法选择我院2013年2月—2014年2月收治的胃癌患者80例为研究对象,采用免疫组化SP法检测患者胃癌组织和癌旁组织中的微血管密度值、RBBP4蛋白水平,采用RT-PCR检测患者胃癌组织和癌旁组织中RBBP4 mRNA水平。术后对患者进行为期6年的随访,并进行Kaplan-Meier生存分析。探讨RBBP4表达水平与患者临床病理参数及生存的相关性。结果胃癌组织中RBBP4 mRNA的表达水平(9.47±2.17)显著高于癌旁组织(1.86±1.98)(P<0.05),RBBP4蛋白阳性表达率(56.25%)显著高于癌旁组织(27.50%)(P<0.05),微血管密度值(42.12±8.72)显著高于癌旁组织(30.54±8.67)(P<0.05);肿瘤呈中低分化、直径≥5 cm、TNM分期为Ⅲ期的胃癌患者RBBP4 mRNA及蛋白水平均显著高于高分化、直径<5 cm和TNM分期为Ⅰ-Ⅱ期者(P<0.05);RBBP4蛋白阳性的胃癌患者微血管密度值(47.32±9.65)显著高于阴性患者(35.43±9.84)(P<0.05);RBBP4蛋白与微血管密度呈正相关(r=0.425,P=0.002);术后患者的平均随访时间为(4.95±1.02)年,RBBP4蛋白表达阳性的胃癌患者总生存率明显低于阴性患者(χ~2=4.869,P=0.021)。结论RBBP4表达水平在胃癌组织中明显增高,且与肿瘤分化程度、直径和TNM分期有关;临床可根据RBBP4表达水平对胃癌患者的预后进行预测,其中RBBP4表达阳性患者预后较差。

Expression Level of Retinoblastoma Binding Protein 4 in Gastric Cancer and Its Clinical Significance

Objective To investigate the expression level,clinical significance and prognosis of retinoblastoma binding protein 4(RBBP4)in gastric cancer.Methods Eighty patients with gastric cancer treated in Huiqiao Medical Center,Nanfang Hospital,Southern Medical University between February 2013 and February 2014 were collected.Immunohistochemical SP method was used to detect the microvascular density and RBBP4 protein levels in gastric cancer tissues and para-cancer tissues.RT-PCR method was used to detect the RBBP4 mRNA levels in gastric cancer tissues and para-cancer tissues.Patients were followed up for six years after surgery for Kaplan-Meier survival analysis.Then the correlation of the expression level of RBBP4 was analyzed with the clinicopathological parameters and the survival of patients.Results The expression level of RBBP4 mRNAwas significantly higher in cancer tissues(9.47±2.17)than in para-cancer tissues(1.86±1.98)(P<0.05).The positive expression rate of RBBP4 protein in cancer tissues(56.25%)was also significantly higher than in para-cancer tissues(27.50%).Moreover,the micro vascular density value in cancer tissues(42.12±8.72)was significantly higher than in para-cancer tissues(30.54±8.67)(P<0.05).The mRNA and protein levels of RBBP4 of patients with middle and low differentiation,tumor diameter≥5 cm and TNM stageⅢwere significantly higher than those of the patients with high differentiation,tumor diameter<5 cm and TNM stageⅠandⅡ(P<0.05).The micro vascular density value of patients with positive expression of RBBP4 protein(47.32±9.65)was significantly higher than that of patients with negative expression of RBBP4 protein(35.43±9.84)(P<0.05).RBBP4 protein was positively correlated with micro vascular density(r=0.425,P=0.002).The mean follow-up time was(4.95±1.02)years.The overall survival rate of patients with positive RBBP4 protein expression was significantly lower than that of patients with negative RBBP4 protein expression(2=4.869,P=0.021).Conclusion The expression level of RBBP4 was significantly increased in gastric cancer tissues,and it was related to the degree of differentiation,tumor diameter and TNM stage.The prognosis of gastric cancer patients can be predicted according to the expression level of RBBP4.Patients with positive expression of RBBP4 protein may have poor prognosis.
