混合核苷片辅助治疗脑胶质瘤放化疗后骨髓抑制的疗效观察

摘    要:
目的观察混合核苷片辅助治疗脑胶质瘤放化疗后骨髓抑制的疗效。方法选择2016年5月—2018年12月我院收治的脑胶质瘤放化疗后骨髓抑制患者60例为研究对象,根据治疗方案的不同分为对照组(31例)和观察组(29例),对照组给予常规治疗,观察组在常规治疗的基础上给予混合核苷片治疗。对比两组患者治疗前后血常规、Notch信号通路相关蛋白、免疫功能变化及骨髓抑制程度。结果两组患者一般资料比较,差异均无统计学意义(P>0.05)。治疗后两组患者血常规指标WBC、PLT、ANC和HGB水平均高于治疗前(P<0.05),且观察组显著高于对照组(P<0.05)。治疗后两组患者jajl、Notch2水平均低于治疗前(P<0.05),且观察组显著低于对照组(P<0.05);numb1、numb2水平均高于治疗前(P<0.05),且观察组显著高于对照组(P<0.05)。治疗后观察组CD8+水平显著低于治疗前(P<0.05),且观察组显著低于对照组(P<0.05);IgA、IgM、NK细胞、CD4+水平均显著高于治疗前(P<0.05),且观察组显著高于对照组(P<0.05)。治疗后,观察组患者0级骨髓抑制比例显著高于对照组(P<0.05)。结论混合核苷片作为辅助用药可改善脑胶质瘤患者放化疗后的骨髓抑制效应,具有临床应用价值。

Abstract:
Objective To observe the efficacy of mixed nucleoside tablets in the adjuvant treatment of myelosuppression after chemoradiotherapy of glioma.Methods A total of 60 patients with myelosuppression after radiotherapy and chemotherapy for gliomas treated in Nanjing Second Hospital between May 2016 and December 2018 were selected as the research subjects.They were divided into two groups according to different treatments.The patients in control group were given conventional treatment(n=31),and those in observation group were treated with mixed nucleoside tablets in addition to conventional treatment(n=29).Before and after treatment,the blood routine test results,Notch signaling pathway-related proteins,immune function,and myelosuppression degree were compared between the two groups.Results There was no significant difference in the general data of patients between the two groups(P>0.05).After treatment,the blood routine test results of WBC,PLT,ANC,and HGB of the two groups were higher than before treatment(P<0.05),and the WBC,PLT,ANC and HGB of the observation group were higher than those of the control group(P<0.05).After treatment,the levels of jajl and Notch2 in both groups were lower than before treatment(P<0.05),but the levels of numbl and numb2 were higher than before treatment(P<0.05).The jajl and Notch2 levels in the observation group were lower than in the control group,but the numbl and numb2 levels were higher in the observation group than in the control group(P<0.05).The CD8+level in the observation group after treatment was lower than before treatment(P<0.05),and was also lower than in the control group(P<0.05).After treatment,the levels of IgA,IgM,and the proportion of NK and CD4+cells in the observation group were higher than before treatment(P<0.05),and were also higher than in the control group(P<0.05).After treatment,the proportion of patients with grade 0 myelosuppression was higher in the observation group than in the control group(P<0.05).Conclusion Mixed nucleoside tablet as adjuvant drug had improving effects on chemoradiotherapy-induced myelosuppression.
