不同剂量喷他佐辛术中静注对呼吸功能影响的观察

目的:探讨喷他佐辛术中静注的合适剂量及其对呼吸功能的影响。方法:60例择期骨科下肢手术病人,ASAⅠ或Ⅱ级,行腰硬联合麻醉,随机分成三组(n=20),切皮前分别静注喷他佐辛10mg(Ⅰ组)、20mg(Ⅱ组)、30mg(Ⅲ组)。记录给药前(T0)及给药后1min(T1)、5min(T2)、10min(T3)、15min(T4)、20min(T5)、25min(T6)、30min(T7)的RR、SpO2、PETCO2、HR、MAP等数值。结果:Ⅰ组、Ⅱ组给药后各时点RR、SpO2、PETCO2变化不大,Ⅲ组SpO2、PET-CO2、RR在T2～T5时点下降较明显,与T0比较差异有统计学意义。结论:小剂量(<20mg)喷他佐辛术中静注比较安全,30mg对病人的呼吸功能有轻度抑制(高峰期在5～20min),应给病人供氧。

Effects of intraoperative intravenous injection of pentazocine at different dose on patient respiratory function

Objective：To determine the appropriate dose of intraoperative intravenous injection of pentazocine by examining the effects of it on respiratory function of the patients.Methods：Sixty adult patients,ASA classⅠ or Ⅱ,underwent selective lower limb surgery under combined spinal-epidural anesthesia and were randomized into three groups（n=20 for each）by administering pentazocine intravenously at dose of 10 mg（Group Ⅰ）,20 mg（Group Ⅱ）and 30 mg（Group Ⅲ）,respectively,before the skin incision.The data were kept with regard to RR,SpO2,PETCO2,HR and MAP before the injection（T0）and after administration at 1 min（T1）,5 min（T2）,10 min（T3）,15 min（T4）,20 min（T5）,25 min（T6）and 30 min（T7）.Results：RR,SpO2,PETCO2 in groupⅠandⅡ changed a little at each time point after administration,while in group Ⅲ SpO2,PETCO2,and RR were decreased significantly at the point T2 to T5,showing statistical difference as compared with T0.Conclusion：Intraoperative intravenous injection of petazocine at lower dose（less than 20 mg）seems safer.Cautiously,when the drug is administered at dose of 30 mg,the patients may experience slight respiratory inhibition,especially at the peak point of 5 min to 20 min,at which oxygen is necessary for the patients.
