膀胱癌术后灌注患者心理健康、社会支持及生存质量研究

目的探讨膀胱癌术后灌注患者的心理健康状况、社会支持情况及生存质量。方法用症状自评量表（SCL-90）、社会支持评定量表（SSRS）及世界卫生组织生存质量量表简表（WHOQOL—BREF）对82例膀胱癌术后灌注患者（灌注组）进行调查分析，并与泌尿科普通就诊患者（普通组）进行比较分析。结果①灌注组在SCL-90中的躯体化、人际关系敏感、焦虑、抑郁、恐怖因子显著高于普通组（P〈0．01）；②灌注组在SSRS中的社会支持总分、客观支持、社会支持的利用度评分显著低于普通组（P〈0．01）；③灌注组在WHOQOL—BREF中的总体评价、生理领域、心理领域、社会关系评分显著低于普通组（P〈0．01）。结论膀胱癌术后灌注患者存在有明显的躯体化、人际敏感、抑郁、焦虑、恐怖等不良心理健康状况，在社会中所获得的支持程度和对支持的利用度均较低，生存质量较差。

Clinical Research on Psychological Status, Social Support and Quality of Life in Patients with Irrigation of Bladder

Objective To explore the mental health status, social support and quality of life in patients with irrigation of bladder. Methods The mental health status, social support and quality of life were evaluated by symptom checklist （SCL-90） , social support rating scale（SSRS） ,and WHO quality of life questionnaire brief（WHOQOL-BREF） in patients suffering from irrigation of bladder（ irrigation group） and ordinary medical patients in urology （ ordinary group）. Results The factor-score of SCL-90 in respect of somatization,interpersonal sensitivity, anxiety, and phobia in the irrigation group was higher than that of the ordinary group （P 〈 0.01 ）. The total scores, the subjective support and social support availability of SSRS among patients with irrigation of bladder were lower than those of the ordinary group（ P 〈 0.01 ）. Scores of physiological field, psychological field and social relationship in the irrigation group were significantly lower than that of the ordinary group（ P 〈 0.01 ）. Conclusion As for patients suffering from irrigation of bladder, there is obvious somatization, interpersonal sensitivity, depression, anxiety, phobia and other undesirable psychological state, meanwhile these patients with poor quality of life gain little social support and have low ability to make use of support from society.
