甲钴胺对断指再植术后感觉功能恢复的促进作用

摘    要:目的探讨甲钴胺对断指再植术后感觉功能恢复的促进作用。方法将236例断指再植术后的病人分为5组,A、B、C组分别肌肉注射5001、000、1 500μg甲钴胺,每日1次,共30次,然后改为每日口服1 500μg甲钴胺片剂,累计共4个月;D组每日口服1 500μg甲钴胺片剂,共4个月;E组为阴性对照组。术后不同时间段对手指感觉功能进行评定。结果 A、B、C、D组在术后不同时间段手指感觉功能恢复均明显优于E组(F=11.473~23.388,q=2.533~5.533,P<0.05),感觉功能恢复的效果与应用甲钴胺的剂量呈正相关(r=0.197~0.322,P<0.05),肌肉注射效果优于口服。长期应用甲钴胺不良反应轻微。结论甲钴胺对断指再植术后断指感觉功能的恢复有促进作用,且与药物的剂量呈正相关。

Abstract:Objective To investigate the effect of methycobal on promoting recovery of sensory function after replantation of amputed fingers. Methods This study consisted of 236 patients(300 fingers) after surgery for digital replantation,who were divided into five groups: A,B,C,D and E.To groups A,B and C,intramuscular injection of methycobal of 500,1 000 and 1 500 μg was given,respectively,once a day for 30 days,and then 1 500 μg/day,orally,for the following 3 months.To group D,1 500 μg/day,orally,for 4 months.Group E served as control group.Sensory function of all the patients was evaluated at different times. Results The recovery of sensory function of groups A,B,C,and D was better than that of the control group(F=11.473-23.388;q=2.533-5.533;P<0.05).The effect was positively correlated with the dosages of methycobal(r=0.197-0.322,P<0.05),intramuscular injection was superior to taking by mouth.The adverse effects were mild with long-term administration. Conclusion Methycobal can promote post-operative recovery of sensory function of digital replantation,and the effect is positively correlated with its dosage.
