晚发VitK缺乏合并颅内出血患儿血清钠离子水平的变化及意义

摘    要:目的 :探讨晚发 Vit K缺乏症合并颅内出血患儿血清钠离子 (Na+ )水平变化与临床的关系。方法 :对 3 8例确诊晚发 Vit K缺乏症合并颅内出血患儿 (出血组 )于入院当天及 3 0例入院普通上呼吸道感染的婴儿 (对照组 )采取静脉血测定血清Na+水平及 Bun、 Cr。结果 :出血组患儿 Glasgows评分 >7分、 5~ 7分、≤ 4分组血清 Na+均低于对照组。 >7分组与对照组比较无显著性差异 (P>0 .0 5) ,5~ 7分组与对照组及 >7分组比较有显著性差异 (P<0 .0 5) ,≤ 4分组与对照组及 >7分组比较有非常显著性差异 (P<0 .0 1)。Glasgows评分越低 ,血清 Na+下降越明显。单纯颅内出血组血清 Na+与合并有脑梗死及缺氧缺血病变组比较有显著性差异 (P<0 .0 5) ,与对照组比较无显著性差异 (P>0 .0 5)。两组测定 Bun、 Cr均正常。结论 :颅内出血患儿病情越重血清 Na+水平下降越明显 ,血清 Na+测定对病情判断及治疗有指导意义

Abstract:Objective:To investigate the r elationship between the change of serum sodium ion(Na +) of the late vitamin K deficiency combined with intracranial hemorrhage children Methods: Take the vein blood of the 38 children diagnosed with the late vitamin K deficient syndrome combined with intracranial hemorrhage(hemorrhage group) at the first day when hospital ized, and collect the vein blood of the 30 infants hospitalized for common upper respiratory infection(control group) Determine the serum Na + ,Bun and Cr in the blood of the two groups Results: The intracranial hemorrhage children were divided into three subgroups according to their Glasgows score: >7, 5~7, and≤4 The serum Na + levels of a ll those three subgroups are lower than that of the control group, and the serum Na + level of the >7 subgroup is of no statistic significance compared with the control group(P>0 05), while those of the 5~7 and the ≤4 of subgroups are of statistic significance compared with both the control group and the >7 subgroup (P<0 05 and P<0 01,respectively) The lower the Glasgows score, the more obvious decline in the serum sodium ion The serum Na + level in the only intracranial hemorrhage group is of statistic significance compared with th e group combined with cerebral infarction and hypoxia-ischemic disease (P<0 05), while it is of no significance compared with the control group (P<0 05) Both groups have normal Bun and Cr levels in the s erum Conclusion:The more serious state of the intracranial hemorrhage, the more evident decline in the se rum sodium ion Therefore,determination of the serum Na +is useful in judging and curing the late vitamin K deficient syndrome
