PPH治疗混合痔外痔处理方法的研究

为探讨痔上黏膜环切术（PPH）治疗混合痔外痔的处理方法，回顾性分析126例混合痔手术患者的临床资料。根据不同手术方式分为PPH组（单纯组，41例）、PPH加外剥内扎组（内扎组，39例）和PPH加单纯外痔切除组（单切组，46例），对3组患者手术时间、住院时间、术后并发症等进行统计分析。结果显示，单纯组、单切组在手术时间、术后早期并发症、住院时间和创口愈合时间方面优于内扎组，而单切组、内扎组复发例数少于单纯组（P〈0．05）。结果表明，PPH加外痔切除不仪去除了外痔，同时也取得较好的近远期效果，是治疗混合痔理想的术式。

PPH Combined with Other Different Procedures for external hemorrhoids of mixed hemorrhoids

This study was to discuss PPH combined with different management for the external hemorrhoids of mixed hemorrhoids,a retrospective study was conducted on the data of 126 cases,which were divided into three groups based on surgical procedure：group PPH （ n =41） ,group of PPH plus external dissection and internal ligation （ n =39） and group of PPH plus surgical removal of external hemorrhoids（ n =46）.All the three groups were statistically analyzed for operation time,hospital stay and postoperative complications.The results showed that group PPH and group of PPH plus surgical removal of external hemorrhoids were superior to group of PPH plus external dissection and internal ligation in operation time, early postoperative complications,hospital stay and wound healing time;while group of PPH plus surgical removal of external hemorrhoids and group of PPH plus external dissection and internal ligation had less recurrence cases than group PPH（ P 〈0.05）.It is concluded that PPH plus surgical removal of external hemorrhoids not only removes the external hemorrhoids,but also has better short-and long-term efficacy, therefore it is an ideal surgical procedure for mixed hemorrhoids.
