环状混合痔PPH术后联合镇痛效果观察

为观察环状混合痔行吻合器痔上黏膜环形切除钉合术（PPH）术后联合镇痛的效果，将86例拟行PPH治疗的环状混合痔患者随机分成治疗组和对照组。各43例，治疗组给予联合镇痛（手术结束前，肛内纳消炎痛栓；术后硬膜外腔接连续型镇痛泵．并根据患者症状追加负荷量）。对照组仅在感觉疼痛时给予曲马多口服，对比两组患者术后肛门疼痛、肛缘水肿、肛门坠胀感情况及不良反应。结果显示，治疗组术后肛门疼痛、肛缘水肿、肛门坠胀感评分均明显低于对照组，P〈0．05。治疗组术后不良反应发生率明显低于对照组，P〈0．05。结果表明，环状混合痔术后联合镇痛可明显减轻患者肛门疼痛、肛缘水肿、肛门坠胀感，且不良反应发生率低．效果满意。

Efficacy of Combined Analgesia after PPH for Circumferential Mixed Hemorrhoids

In order to observe the efficacy of combined analgesia after PPH for circumferential mixed hem- orrhiods,authors randomly divided the 86 patients with this disease who received PPH into treatment group and control one, n = 43 for each group; gave to treatment group the combined analgesia, i.e before ending operation via anus administering Xiaoyantong suppository, and a{ter operation connecting epidural cavity and continuous type analgesia pump, and adding loading dosage according to patient＇s symptom; while to control group only oral Tramadol when pain; then compared the anal pain, edema and anal te- nesmus,as well as adverse reaction after operation between both groups.As results,the scores on the first- three items and the incidence of adverse reaction of treatment group were significantly lower than that of control group（ P〈0.05）.Results show that above-mentioned combined analgesia can significantly reduce the pain,edema and anal tenesmus and with lower incidence of adverse reaction,thus outcome is satisfacto- ry.
