小儿肺炎合并全身性炎症反应综合征时高代谢状态研究

摘    要:目的探讨血糖、血乳酸在合并全身性炎症反应综合征(SIRS)的小儿肺炎患者中的变化.方法选择83例肺炎合并SIRS患儿分别进行血糖、血乳酸和血气指标的测定,同时选择非SIRS的肺炎患儿和正常健康儿童作为对照组,以观察这些指标的动态变化.结果小儿肺炎患儿SIRS发生率为58.5%;SIRS组血糖和血乳酸值与对照组比较具有显著性差异(P<0.01或P<0.05),但血气分析结果比较差异无显著性.结论小儿肺炎患儿SIRS发生率高,SIRS时血糖、血乳酸等高代谢指标显著增高.

Abstract:Objective: To determine the change of blood glucose and blood lactic acid in children with pneumonia accompanying systemic inflammatory response syndrome (SIRS).Methods: Patients with pneumonia were divided into two groups: SIRS group (S group) and non-SIRS control group (NS group). Healthy children were for normal control (N group). Blood glucose, lactic acid, PaCO 2 and PaO 2 were evaluated in all the children. Results:About 58.5% of the children with pneumonia had accompanying SIRS. The blood glucose and lactic acid in S group were significantly increased than in the control groups (P<0.01 or P< 0.05). However, blood gas analysis showed no significant difference between S group and NS group. Conclusions:The high-metabolic parameters (blood glucose, blood lactic acid, etc) were significantly increased in children with SIRS accompanying pneumonia.
