细菌感染性疾病患者白细胞VCS参数的变化及临床意义

目的探讨细菌感染性疾病患者白细胞(WBC)体积、浆核比例、胞浆颗粒的变化及其临床意义。方法利用Beckman-Coulter Gen.S全自动血液分析仪的体积、高频传导、激光散射(VCS)技术测定69例细菌感染性疾病患者(其中肺部感染53例、胆道感染9例、颅内感染7例)及72名健康人的WBC总数和中性粒细胞体积(NEV)、浆核比例(NEC)、胞浆颗粒特性(NES)和淋巴细胞体积(LYV)、浆核比例(LYC)、胞浆颗粒特性(LYS)及单核细胞体积(MOV、)浆核比例(MOC)、胞浆颗粒特性(MOS)以及他们各自的平均标准差(s)。结果与正常对照组比较,中性粒细胞(NE)、淋巴细胞(LY)和单核细胞(MO)的体积及光散射量明显增大(P<0.01或P<0.05),而浆核比例均明显降低(P<0.01);除MOS-s与对照组差异无统计学意义(P>0.05)外,其余各参数的s均明显大于正常对照组(P<0.01或P<0.05)。结论细菌感染可使外周血WBC核及体积增大,胞浆颗粒增多;检测WBC的VCS参数可作为了解细菌感染程度的辅助指标之一。

The change and clinical significance of the white blood cell volume and plasm-nucleus complexity in patients with bacteria infection

Objective To explore the change and the clinical significance of the white blood cell(WBC) volume,the ratio of plasm-nucleus and the plasm's granule in the patients with bacteria infection. Methods Using the technique of volume,conductivity and scatter(VCS) of the automatic blood cell analyzer(Beckman-Coulter Gen.S),the WBC and neutrophil's volume(NEV),plasm-nucleus's ratio(NEC),plasm granule's characteristic(NES),the lymphocyte's volume(LYV),ratio of plasm-nucleus(LYC),the characteristic of plasm granule(LYS),the monocyte's volume(MOV),plasm-nucleus ratio(MOC),the plasm granule's characteristic(MOS) and their average standard deviation(s) of 69 patients with bacteria infection(including 53 lung infection,9 gallbladder infection and 7 encephalic infection) and 72 normal subjects were determined. Results Compared with the normal group,the volume and the light scatter of neutrophilic(NE),lymphocyte(LY),monocyte(MO) increased significantly(P<(0.01) or P<(0.05)),but the ratio of plasm-nucleus falled significantly(P<(0.01));whereas MOS-s was not significant(P>(0.05)),the rest parameters' s were significantly higher than the normal group(P<(0.01) or P<(0.05)). Conclusions In bacteria infection,the WBS's nucleus and volume will be larger and the plasm granules will augment in the peripheral blood.The parameters of VCS can be used as an indexe of the severity of bacteria infection.
