爱全乐联合长期家庭氧疗对慢性阻塞性肺病患者的疗效分析

目的探讨爱全乐联合长期家庭氧疗（LTOT）对改善慢性阻塞性肺病（COPD）患者的生活质量疗效。方法将临床缓解期COPD患者随机分为2组,治疗组每日鼻导管吸氧15h以上,同时每天吸入爱全乐气雾剂2次,每次2喷;对照组仅每日鼻导管吸氧15h以上。比较两组1、2、3年后呼吸困难指数、住院次数、血气分析（血氧分压）、肺功能（FEV1）、体重指数（BMI）、6min步行路程的变化。结果两组治疗前后各项指标均有改善,差异均有统计学意义（均P〈0.01）。治疗组与对照组FEV1比较差异无统计学意义（P〉0.05）,但住院次数、呼吸困难指数、血氧分压水平、体重指数、6min步行路程比较差异均有统计学意义（均P〈0.05）。结论爱全乐联合长期家庭氧疗较单纯长期家庭氧疗更能改善COPD患者的生活质量。

Therapeutic effect of ipratropium bromide combined with long-term domiciliary oxygen therapy for patients with chronic obstructive pulmonary disease

Objective To observe the therapeutic effect of ipratropium bromide combined with long-term domiciliary oxygen therapy(LTOT)for patients with chronic obstructive pulmonary disease(COPD).Methods Thirty COPD patients in clinical remission period were randomly divided into treatment group(ipratropium bromide combined with LTOT,n=30)and control group(LTOT only,n= 20).Both groups received oxygen therapy for 15 hours or more per day,treatment group received inhaled ipratropium bromide aerosol twice a day,2 sprays each time.Borg Scale,hospitalization frequency,blood gas analysis,values of PaO2,pulmonary function(FEV1),body mass index(BMI)and six-minute walk distance were examined annually for 3 years.Results All the indications in both groups were significantly improved after treatment(P<0.01).The differences in hospitalization frequency,Borg Scale,PaO2,BMI and six-minute walk distance between two groups were significant(P<0.05).Conclusion Ipratropium bromide combined with LTOT is better in improving quality of life in COPD patients than LTOT alone.
