晚上睡前拔除尿管并会阴冲洗促进排尿的实践与效果

目的：探讨患者晚上睡前拔除尿管并行会阴冲洗促进自行排尿，减少尿潴留的发生，减轻患者的痛苦。方法：观察组先夹放尿管训练膀胱功能，当患者晚上睡前膀胱充盈有尿意时，抽出气囊内液体拔除尿管，然后用洁尔阴溶液冲洗会阴及尿道口，对照组同样先夹放尿管训练膀胱功能，白天患者有尿意时用常规方法拔除尿管。观察：两组患者第1次排尿情况及首次排尿时间，并对2组患者的再次导尿率和拔管后自我舒适程度进行评定。结果：两组患者自行排尿情况和首次排尿时间以及拔管后自我舒适程度有显著性差异（P〈0．01）。结论：晚上睡前拔除尿管并会阴冲洗，提高了患者的舒适度，促进了自行排尿，减少了尿潴留的发生，减轻了患者的痛苦。

Methods：Catheter placed first folder observation group of bladder training function, The evening before going to bed when the patient has to urinate when the bladder filling, removal of fluid out of balloon catheter,Jreeryin solution and then rinse the perineum and the urethra, The same control group, training the bladder catheter placed first folder function, patients have to urinate during the day when the catheter removal by conventional methods. Observed： 1st 2 groups of patients and the first voiding time, voiding the situation, and 2 groups of patients re-catheterization rates and after extubation were assessed self-comfort. Results ： 2 groups of patients urinate urination first time and the circumstances and self-comfort after extubation were significantly different （P 〈0.01）.Conclusion： Catheter removal and before bed at night washing perineum, improved patient comfort, and promote the self-voiding, reduced the incidence of urinary retention, reduce the patient＇s pain.
