摘    要:目的:为提高宫腔内人工授精(IUI)妊娠率,降低流产率。方法:本文对40例患有排卵障碍、轻度输卵管因素不孕、免疫性不孕、原因不明性不孕等妇女,在促排卵药物处理后行卵泡刺破同时行IUI共 47个周期。结果:生化妊娠 18例,占 45%;临床妊娠 16例,占40%,周期妊娠率为34%;2例于妊娠6周自然流产,流产率占5%;2 例已分娩获得活婴,继续妊娠12例(其中三胞胎一例)。结论:本组行卵泡刺破加IUI的患者均为hCG注射36h后经阴道B超观察至40 h卵子未排出者,在阴道B超引导下,适时刺破卵泡排出多个卵子,避免卵子因延迟排出而老化,从而提高排出卵子的质量和数量,提高妊娠率。

Abstract:Objective: To improve the pregnancy rate of intrauterine insemination, and decrease abortion rate. Methods: 40 cases with anovulation, light infertility caused by fallopian tubes factors,immunological infertility and unidentified infertility etc., were accepted the operation of follicle puncturing after superovulation medical treatment and intrauterine insemination for 47 cycles.Results: Biochological pregnancy rate was 45 % (18 cases ), clinic pregnancy rates was 40% (16 cases) and cycle pregnancy rate was 34%. Two cases of abortion occurred 6 weeks after pregnancy. The abortion rate was 5%. Two cases have dilivered healthy babies and 12 cases are of ongoing pregnancy (one is triplet). Conclution: In these patients, follicle puncturing and intrauterine insemination were carried out 36 h after injection of hCG and follicles were not found until 40 hours, by transviginal ultrasound. Under the direction of transviginal ultrasound, puncturing follicles could release more oocytes at the same time, increase the chance of fallopian tube picking up oocytes, and prevent the aging of mature oocytes induced by prolonged releasing. This method could improve the quality and quantity of oocytes, and improve pregnancy rate.
