腰椎间盘突出症两种手术方案的临床疗效比较

目的评价治疗腰椎间盘突出症的显微内镜椎间盘切除术（MED）和开窗手术（OLD）两种方案的临床疗效。方法比较38例MED手术患者与42例OLD手术患者的手术时间、术中出血量、术后住院日、术后并发症、手术疗效以及治疗费用6项疗效指标。结果MED组术中出血量、术后住院日均低于OLD组，而治疗费用高于OLD组，且差异有统计学意义（P〈0．05）；手术时间、手术疗效、并发症例数在2组中差异无统计学意义（P〉0．05）。结论MED手术创伤小，术后恢复快；OLD手术适应症广泛、费用低。临床医师可根据患者的身体情况和经济条件，合理选用术式。

Comparison of Clinical Effects between Two Operation Plans in Treatment of Lumbar Intervertebral Disc Protrusion

Objective To evaluate the clinical effects of microendoscopic discectomy （MED）and open lumbar discectomy（ OLD ） in treatment of lumbar intervertebral disc protrusion. Methods 6 indexes were compared between 38 cases of MED and 42 cases of OLD, including the duration of operation, volume of blood loss, admission days after operation, postoperative complication, operative efficacy and treatment expenditure. Results In MED group,the results of bleeding volume in operation and admission days after operation were less than those in OLD group, but the treatment expenditure was higher than that in OLD group,indicating statistic significant difference（ P 〈 0.05 ）. There were no significant differences in the duration of operation, operative efficacy and the eases of complication between two groups statistically （ P 〉 0.05 ）. Conclusion MED results in small injury in operation and the postoperative recovery is quick. OLD has extensive indications and low cost in expenditure. The clinical physicians may option the appropriate operation according to the physical and economic conditions of patients.
