摘 要: | 目的:分析肝肾间隙巨大肿瘤病灶在MSCT上的影像特征,提高MSCT对该部位病灶定位诊断的准确性。方法:回顾性分析经临床手术与病理证实的26例具有完整资料的肝肾间隙巨大肿瘤患者,通过对肿瘤在MSCT轴位与在后处理MPR、VR两种图像上的影像表现进行对照分析,并同手术及病理结果相比较,分析MSCT后处理MPR与VR对肝肾间隙巨大肿瘤病灶的定位优势及对肿瘤定位有价值的影像表现。结果:26例肝肾间隙巨大肿瘤,其中CT轴位定位诊断:肝脏来源8例,右肾来源6例,肾上腺来源7例,腹膜后其它组织来源5例。MSCT后处理MPR与VR相结合诊断:6例来源于肝脏,6例来源于右肾上极,9例来源于右侧肾上腺,5例来源于腹膜后其它组织。同手术与病理结果(肝脏来源6例,右肾来源7例,右肾上腺来源8例,腹膜后其它组织来源5例)对照,CT轴位定位准确22例,占85%;CT轴位与MPR、VR相结合,定位准确25例,占96%;两种方法对照,差异有统计学意义(P<0.05)。结论:肝肾间隙巨大肿瘤的MSCT表现具有一定的特征性,MSCT后处理MPR与VR相结合对肝肾间隙巨大肿瘤的定位诊断具有重要优势及临床实用价值。

Abstract: | Objective:To analyze the imaging features and improve the diagnostic accuracy of massive tumor in hepatorenal space by MSCT.Methods:26 cases of massive occupying lesion in hepatorenal space confirmed by surgery and pathology were retrospectively analyzed.By contrastly analyzing the imaging data of tumor with MSCT axial and post-processing techniques MPR and VR,and by comparing with the results of surgery and pathology,the superiority of localization of massive tumor in hepatorenal space and the valuable imaging characteristics of helping localization were assessed.Results:26 cases of massive occupying lesion in hepatorenal space were defined by CT axial:8 from liver,6 from right kidney,7 from adrenal gland,and 5 from the retroperitoneal other origins.By combining CT axial and post-processing techniques with MPR and VR,the lesions were 6 from liver,6 from right kidney,9 from adrenal gland,and 5 from the retroperitoneal other origins.Contrasting with the results of surgery and pathology,22 cases were accurately localized by CT axial,and the accurate rate was 85%, 25 cases were accurately localized by combining CT axial and MPR and VR,the accurate rate was 96%.Therefore, there exists statistical significance in those two different methods(P<0.05).Conclusion:The CT performance of tumor in hepatorenal space has certain imaging characteristic,the post-processing techniques combined with MPR and VR of MSCT has superiority and clinical value to the localization diagnosis of massive tumor in hepatorenal space. |
