经睑结膜内入路结合上龈颊沟切口整复单纯颧骨骨折

目的:探讨睑结膜内切口结合上龈颊沟切口整复单纯颧骨骨折的临床效果。方法:对13例单纯颧骨骨折患者,采用经下睑结膜内、上睑结膜外侧穹窿联合口内上龈颊沟的微创隐蔽切口显露骨折部位,复位后用微型钛板行坚固内固定。术后1～6月评价临床治疗效果。结果:术后伤口均Ⅰ期愈合,除1例复杂骨折患者外形恢复欠满意外,其余患者面部外形恢复满意,双侧颧面部外形对称,张口度及咬合关系正常,眶下区麻木及复视消失,愈后良好。结论:睑结膜内切口结合上龈颊沟切口是一种隐蔽微创的切口。与传统冠状切口和目前应用较多的面部小切口相比,具有创伤小、恢复快、并发症少、面部无瘢痕等优点,是治疗单纯颧骨体骨折较为理想的入路。

The application of conjunctiva approach combined with superior buccal gingival sulcus incision treating simple zygomatic complex fracture

Objective: To explore the clinical effect of conjunctiva approach combined with superior buccal gingival sulcus incision in treatment for simple zygomatic complex fracture.Methods: The clinical data of 13 patients with simple zygomatic complex fracture were analyzed retrospectively.According to fracture site and type,reduction and rigid internal fixation of fracture was performed by conjunctiva approach combined with superior buccal gingival sulcus incision.After 1～6 months,the clinic effects were evaluated.Results:All the incisions closed up in one stage.Except one patient with composite fractures,12 cases healed well without any severe complications and regained satisfactory occlusion and pleasing appearance.Infraorbital numbness and diplopia disappeared in 3 months post-operation.Conclusion:Compared with traditional coronal or semi-coronal and facial local incision,this approach is a safe method with short hospitalization,low incidence rate of complication,slight trauma,rapid recovery and no facial scar.It is the ideal choice to treat simple zygomatic complex fracture.
